本研究では、特殊操作を要する据置型コンピュータを必要とし、これまで診療現場では容易に利用できなかった医用画像を含む臨床検査マルチメディア・データベースを、ユーザーインターフェース機能に優れた携帯型パソコン(Macintosh PowerBook Duo280c)にソフト上の工夫を凝らして詰め込み、機動性と操作性を飛躍的に向上させ、さまざまな立場の専門医および臨床医による臨床的有用性の評価に供した。1.格納作業において、携帯型パソコンに特有のハードウエアの制約には概ね対応可能であったが、補助記憶容量の限界(320メガバイト)に著しい制約を受けた。そこでデータ構造の簡素化と格納データの厳選により、臨床評価に耐える最小規模のデータベースの格納に成功した。2.評価期間中耐久性の問題はなかったが、重量(2.2kg)と形状(閉じた状態で277×216×38mm)、蓄電池容量(連続稼動2〜4時間)には改善が求められた。3.今回採用した携帯型パソコンは処理能力の向上が著しく(CPU:MC68LC040 66/33MHz)、検索応答時間について不満はなかった。4.携帯型パソコンの液晶表示装置(8.4インチTFT・640×400ドット・32,768色表示)の画質は、一部の医用画像では専門医の診断用途には不適とされたが、患者説明・症例報告・医学教育等には十分との評価を得た。表示サイズの制約は拡大・縮小・スクロール等の機能充実により克服可能であった。5.臨床的有用性につき、理由は種々であるが肯定的な意見が大半を占めた。特に動画像参照のニーズが強かった。また医用画像を施設間の患者紹介に使えるよう、フォーマットの標準化を切望する積極的意見もあった。今後は以上の知見を元に、ダウンサイジングの伸展とセットワーク技術の活用で補助記憶容量の制約を克服し、さらに実用化の可能性を追求する計画である。
