高齢者の下腿にドライスキンとともによく見られる貨幣状湿疹の病因、病態を検索した。代表的な環境抗原であるダニ、ハウスダスト抗原でパッチテストを行ったところ、高齢者の貨幣状湿疹患者は正常の高齢者に比して高率に遅延型の接触過敏反応が惹起された。これは正常の若年者に比しても高率であった。ただし、ダニ、ハウスダスト抗原に対する血清IgE値の上昇は伴わなかった。カンジダ抗原に対しても高齢者の貨幣状湿疹患者では、通常、加齢とともに陽性率が低下するにもかかわらず、若年者と同様の高率に接触過敏反応を呈した。これらから、高齢者の貨幣状湿疹患者では、若年者のアトピー性皮膚炎の様な血清IgE値の上昇は伴わずにダニ、ハウスダスト抗原に対する遅延型の接触過敏反応が高率に成立していることが分かった。カンジダ抗原に対するような、基本的な抗原に対する遅延型反応の結果からも高齢者の貨幣状湿疹患者では免疫能がむしろ維持されている可能性が示唆された。非侵襲的な検査法による皮膚のバリアー機能を経表皮水分喪失量を指標として測定すると、高齢者の貨幣状湿疹患者の下腿伸側の無疹部と正常の高齢者比較すると、経表皮水分喪失量は前者で高値を示した。角層水分量は前者が低値であり、高齢者の貨幣状湿疹患者の皮膚は無疹部においてもバリアー機能が低下していることが分かった。しかし、皮表脂質の量は差を認めなかった。高齢者の貨幣状湿疹は環境抗原に対する接触過敏反応と皮膚のバリアー機能が低下の両者が関与して発症すると考えられた。

The causes and pathologies of monetary eczema are discussed in detail below. Representative environmental antigens are more likely to cause allergic reactions in monetary eczema patients than in normal individuals. The rate of death is higher than normal. The serum IgE level was increased by 100%, 100%. The positive rate of eczema is usually lower than that of normal eczema, and the positive rate of eczema is higher than that of normal eczema. The increase in serum IgE levels in patients with monetary eczema, especially those with high levels of eczema, is accompanied by a high rate of delayed contact allergy in patients with chronic dermatitis. The results of the delayed response to the primary antigen showed that there was a possibility that the immune function could be maintained in patients with monetary eczema. Non-invasive skin tests showed that skin moisture loss was higher in patients with monetary eczema than in patients with normal eczema. The moisture content of the horn layer is lower than that of the former, and the skin of the monetary eczema patient is lower than that of the former. The amount of lipid on the skin surface is different. The relationship between high fever and low skin function is discussed.
