PTFE(polytetrafluoroethylene)covered Spiral Z-stentの静脈内留置後の変化を検討するとともに抗凝固剤の併用による初期血栓の抑制と開存性の向上が得られるか否かを検討した。抗凝固薬非投与群(2週6頭、4週6頭、12週6頭)とシロスタゾール+ワ-ファリン投与群(4週3頭)、シロスタゾール単独投与群(2週3頭、4週4頭)、ワ-ファリン単独投与群(4週3頭)の計31頭の成犬(体重12.2〜17.0kg)の下大静脈にPTFE Spiral Z-stentを留置した。Stent留置手技中のみに1000IUのヘパリンを投与した。抗凝固薬投与群は、stent留置2日前より屠殺当日まで投薬した。抗凝固薬非投与群は、2週後にstent全体に血球成分よりなる表面不整な血栓(初期血栓)付着を認め、両端部に器質化部分がみられた。4週後には、両端部で血栓の器質化がみられ、12週後では、ほぼ全体の器質化がみられた。PTFEから内腔面までの距離(内膜肥厚)は、経過観察期間中に有意差を認めなかった。走査電子顕微鏡では、2週後にstent両端部に部分的な内皮化がみられたが、中央部には血栓付着がみられた。4週後には、ほぼ全体が内皮化され、12週後には、完全な内皮化がみられた。シロスタゾール+ワ-ファリン投与群では、優位な内膜肥厚の抑制が認められた。シロスタゾール単独投与群は、抗凝固薬非投与群と同様の内膜肥厚が認められた。ワ-ファリン単独投与群は、実験進行中である。今回の実験結果より、PTFE Spiral Z-stent内腔は、経時的に両端から中央に向かって初期血栓の器質化と内皮化が進むと考えられた。また、経時的な内膜肥厚の増加傾向が無いことから、内腔狭窄の程度は初期血栓量に依存すると考えられ、適切な抗凝固薬の投与により初期血栓の付着抑制が得られ、PTFE Spiral Z-stentが長期開存することが期待できる。
