歯状核赤核淡蒼球ルイ体萎縮性(DRPLA)は常染色体優性遺伝を示し、小脳歯状核系と淡蒼球外節ルイ体系に病変の主座をもつ多系統変性症である。臨床的には小脳失調、舞踏病様運動、ミオクローヌス等の神経症状とともに痴呆や人格変化がしばしば出現し、皮質下痴呆をきたす代表的疾患の1つとされている。しかし従来DRPLAの痴呆を含む精神症状に関する報告は少なく、その臨床的特徴は明らかではない。また、DRPLAの痴呆の責任病巣は皮質下病変に基づくだけのものとは考えにくく、本症に認められる画像所見や脳萎縮、脳重量の減少から大脳皮質、大脳白質を含む広範な障害が推察されるが、神経病理学的には変化に乏しくこの矛盾点は解明されていない。今回我々は、DRPLAの2症例を経験し、痴呆や人格変化、精神症状に着目し、その特徴について精神医学的、画像診断学的に検討を加えた。症例1は46歳女性。小脳性運動失調で発病し、舞踏病アテト-セ様付随意運動、ミオクローヌス、痴呆に加え幻視、幻聴、被害妄想、精神運動興奮などの精神分裂病様の症状をはじめとする多彩な精神分裂病様症状を呈し、臨床的には明らかな遺伝歴をもたず、第12染色体短腕のCAG repeatの延長を確認することによってDRPLAと診断できた。DRPLAの遺伝子異常が発見されてまもないため、本例のように遺伝子解析で確認された孤発例の報告は少なく興味ある例であった。症例2は47歳男性。明らなか遺伝歴を持ち、舞踏様アテト-ゼ様不随意運動、小脳性運動失調で発病し、遺伝子解析でCAG repeatの延長を認め、経過途中で幻聴、幻視が出現した1男性例である。従来、ハンチントン病と比べDRPLAの精神分裂病様症状についての報告は少ない。本例の分裂病様症状は被害的多彩が少なく体系化された妄想は認めず、割合と単純な内容であり、症状が変動しやすいという特徴を持っていた。CT,MRI,SPECT等の画像学的所見や遺伝子解析における特徴と精神症状との関連について検討し報告した。さらに、本例の痴呆や精神症状の特徴について検討した。今後、神経病理学的所見に精神症状との関連について検討を加える予定である。
