本研究は高齢者の頭蓋内出血例の転帰不良となる因子、特に脳の脆弱性の指標としてamyloid angiopathy(以下AA)の存在を重要視し、開頭手術を施行した高齢者頭蓋内出血例において病理学的にAAの存在を検索し、臨床像との比較検討を行なった。対象は1994年6月より1995年1月まで当院に入院し、脳動脈瘤破裂に伴うくも膜下出血、高血圧性脳内出血、外傷性頭蓋内出血と診断され、それに対して開頭術が成された60歳以上の高齢者例とした。内訳はくも膜下出血(7例)、脳内出血(小脳出血2例、被殻出血1例)、外傷性頭蓋内出血(1例)の計11例で年齢は60-92歳にわたり、平均69歳であった。また、60歳以上の慢性硬膜下血腫例に対する穿頭術に際し、硬膜の採取可能であった12例に関しても同様な検討を行なった。AAの存在の有無は病理所見により診断することとし、摘出標本は症例の転帰に全く影響を与えない髄膜(大多数が硬膜、くも膜の採取は多くの例で脳の侵襲の大きさを考慮し困難であった)とした。標本として髄膜をCongo-red染色し、髄膜血管のアミロイド沈着、Tunica mediaの破裂等の有無を観察した。また、頭蓋内出血における臨床、神経放射線学的所見とを対比検討し、高齢者頭蓋内出血例におけるAAの関与性を検討した。結果は、1)発症以前に痴呆症が存在する、2)CTscan上の血腫の形状が大葉性、3)明らかに容易な手術操作で脳内血腫が発生したなどの臨床的にAAの存在が疑われたのは3例であった。しかし、これらを含め11例全例において病理学的なAAの存在は証明されなかった。一方、慢性硬膜下血腫例においては、穿頭術による採取した硬膜であるため少量で、且つ手術による焼灼操作により挫滅破壊が著しく、全例不十分な標本作成となった。高齢者の開頭術の適応は限定され症例数が少ないが、症例を積み重ねるとともに、剖検例での検討も重要と考え、今後継続研究していく予定である。

This study investigated the presence of amyloid angiopathy(AA) in intracranial hemorrhage cases in patients with hypertension, the importance of factors affecting the severity of intracranial hemorrhage, the presence of AA in the pathology of intracranial hemorrhage cases in patients with hypertension, and the comparison of clinical findings. From June 1994 to January 1995, the diagnosis of subcapsular hemorrhage, hypertensive intracerebral hemorrhage and traumatic intracerebral hemorrhage was made in patients admitted to the hospital with aneurysm rupture. There were 7 cases of internal bleeding under the diaphragm, 2 cases of internal bleeding under the diaphragm, 1 case of internal bleeding under the diaphragm and 1 case of traumatic internal bleeding under the skull. For chronic subdural hematomas over 60 years of age, there are 12 cases of subdural hematomas. The presence of AA is not diagnosed by pathology, and the presence of AA is not diagnosed by pathology. Congo-red staining of the membrane and the presence or absence of vascular breakdown and Tunica media rupture were observed. Clinical and neuroradiological findings of intracranial hemorrhage were compared and discussed. Results: 1) Dementia existed before onset; 2) Hematoma shape on CT scan was lobular; 3) Hematoma was easy to operate on; 4) Clinical AA existed in 3 cases. The existence of AA in 11 cases was proved by pathology. One side, chronic subdural hematoma case, piercing surgery, taking a small amount of dura, and surgery, cauterization, all cases are not very good. The initial procedure of the patient is limited to the number of cases, the accumulation of cases, the investigation of cases, and the determination of future research.
