本研究は、介護従事者による介護プラン作成を支援する事例ベース型知識システム構築の基礎として、介護ニーズと介護力のアセスメントに基づき介護プランを作成する過程での介護事例の利用方法を設計し、排泄介護を対象としてその設計に基づくプロトタイプシステムを作成することを目的に行われた。本研究の成果は以下のとおりである。まず、介護プランを記述する介護アクティビティを階層的に整理し、介護プランを介護アクティビティの集合により表現する枠組みを構築した。介護アクティビティは、大きく動作援助、生活リズム調節、心理的援助、介護サービス提供、介護用具紹介、住宅改造、疾患に対する配慮のアクテビティグループにまとめられた。次に、介護事例をアセスメントと介護アクティビティを軸として記述する方法を構築し、ヒアリングにより収集した事例をこの方法により再記述することを通じて本方法の有効性を確かめた。また、介護事例のインデクスとして、介護アクティビティを誰が・いつ・どこで遂行したか、介護アクティビティを特徴づけるキーワードは何か、及び特に根拠としたアセスメント項目は何か、を採用し、これらを用いて介護事例を検索する仕組みを設計した。さらに、アセスメント機構、プラン作成機構、事例検索機構を主構成要素とする排泄介護プラン作成支援のための事例ベース型知識システムのプロトタイプを試作した。介護経験10年以上の介護専門家による本格的評価は計画通りに行えなかったが、若干名の専門家から基本的機能について実用化しうるシステムであるとの評価を得ている。今後、本システムを実際の介護の場で試用することを通じて、介護プラン作成過程における介護事例の利用方法を改善して行くことが課題として残されている。
