To verify the usefulness of a new embolic material, cellulose acetate polymer (CAP), we performed following experimental studies.(1) Twenty-four experimental canine lateral wall aneurysms were thrombosed with a CAP solution. Angiography performed 1 to 4 weeks after CAP injection revealed complete thrombosis of the aneurysms with patency of the parent artery in 16 aneurysms. Histological analysis disclosed the aneurysmal orifice was completely covered with newly formed endothelial cells 2 weeks after CAP thrombosis.Time until complete reendothelialization was shorter than that described in reports with other embolic materials.(2) We created 9 canine carotid bifurcation aneurysms, which were intentionally partially thrombosed with CAP solution. Follow-up angiograms of 7 aneurysms without crescent crevice between the CAP mass and the aneurysmal sac revealed no shifts of the positions of the CAP masses. Histologically, 7 aneurysms without enlarged remnant had a newly developed endothelium between the aneurysmal lumen and the CAP mass.Partial thrombosis with the CAP solution could prevent regrowth, unless crescent crevice had been left.(3) We used interlocking detachable coils (IDCs) for creating an intraaneurysmal frame. We created 11 canine cervical carotid bifurcation aneurysms. Complete thrombosis was attempted in six cases and intentionally partial thrombosis in five cases. Direct thrombosis of the aneurysm with IDC and CAP was performed. The follow-up angiography was performed 1,2,4 and 8 weeks after thrombosis. In the complete thrombosis group, all aneurysms were successfully thrombosed. Aneurysmal enlargement, however, was demonstrated in two of partial thrombosis group. There were no compaction of IDC-CAP complex.This method can be a new useful technique for the treatment of cerebral aneurysms.Our studies demonstrated that CAP solution is a safe and useful embolic material for thrombosis of aneurysms.
