本研究の目的は,ブロ-ネンマルクインプラントにより無歯顎患者の咬合咀嚼機能が回復していく経時的な動態を究明し,従来の他の補綴修復処置による咬合咀嚼機能回復との比較検討を行い,上部構造及び咬合関係との関わりを明らかにすることにある.当科において骨結合型チタンインプラント(ブロ-ネンマルクシステム)を用いて歯科補綴処置がなされた下顎無歯顎患者3名,部分的歯牙欠損患者4名を被験者として,インプラント治療前及び治療後の咬合咀嚼機能の回復の評価を行った.1)咀嚼筋前部と咬筋中央部の咀嚼筋筋電図分析:無歯顎患者では,噛みしめ時ならびにタッピング時における筋放電活動は,左右側で対称性に行われていた.部分的歯牙欠損患者では,インプラント上部構造を装着することにより,患側(補綴側)の咬筋中央部の咀嚼筋筋放電活動が賦活化された.2)咀嚼能率試験(Manly法)の結果:無歯顎患者では正常人の約51%,また部分的歯牙欠損患者では正常人の約94%に回復していた.3)瞬時最大咬合力の測定結果:無歯顎患者では大臼歯部において最大値48kgを記録した.4)MKGによる顎運動分析の結果:無歯顎患者では,習慣性開閉口運動や側方滑走運動が非常に円滑であり,健康有歯顎者と類似した顎運動路であった.部分的歯牙欠損患者では,インプラント上部構造を装着しても患者本来の顎運動を干渉することはなかった.5)顎関節断層撮影による顎関節の検索結果:顎関節断層X線写真では,被験者の全てが良好な顆頭位に回復されており,また下顎頭の動きも円滑であった.

The purpose of this study is to investigate the dynamics of occlusal mastication recovery in patients without teeth, to compare occlusal mastication recovery in patients with other prosthetic treatments, and to clarify the relationship between superstructure and occlusal relationship. 1) Evaluation of the recovery of masticatory function before and after treatment. 2) Analysis of the electrical activity of masticatory muscles in the central part of anterior masticatory muscles. 3 patients without mandibular teeth and 4 patients with partial dental defects. Some of the patients with dental defects have upper structures installed on the affected side (Patch side) The chewing ribs in the central part of the chewing ribs are activated. 2) Chewing energy test (Manly method) Results: 51% of patients with no teeth recovered from normal people, and 94% of patients with partial tooth loss recovered from normal people. 3) Measurement results of instantaneous maximum occlusal force: Results of jaw movement analysis in patients with no teeth: Habitual open-close movement and lateral sliding movement in patients with no teeth, and similar jaw movement in healthy patients. Some of the patients with dental defects were found to have a good upper structure and a good jaw movement. 5) The results of the examination of the jaw joint were as follows: The jaw joint X-ray photographs were all good and the jaw head was restored.
