保健所保健・福祉サービス調整推進事業は、保健・医療・福祉の連携強化を図ることで、より充実した保健福祉体制を確立することを目的としている。そこで今回、地域のケア支援体制づくりの観点から、鹿児島県(鹿児島市を除く)における調整推進事業の実施状況(平成3〜6年度)および今後の課題を明らかにするため、事業報告書の内容分析および保健指導係長への面接聞き取り調査を行い、以下の点について検討したので報告する。1)会議は、看護や福祉関係の実務者レベルの出席が多く、互いに連絡・連携を図っている現状が認められたが、更に組織の管理職や地域の非専門職を巻き込んで実施する必要性が示唆された。また、各保健所における会議テーマ・内容の継続性・発展性を経年的に分析したが、5保健所における6テーマの実施に止まっており、会議の発展性を考慮した計画的取組の必要性が明らかになった。2)事例検討は、処遇検討に止まらず、当該事例の問題や課題を地域のこととして一般化させ、その共有化と互いの認識確認を意図しての実施は、全体の1割と少なかった。また、家族や地域の実情を代弁する家族会の代表や自治会長、警察・消防関係など地域を動かすキ-・パーソンの出席は非常に少なく、意図的に出席者として組み入れる必要性が示唆された。3)今度、地域のケア支援体制づくりを効果的に進めるためには、事例検討と会議の機能役割を明確化した上で、事例検討から抽出された地域の課題を会議のテーマとするなど、会議と事例検討を連動させた一連の事業システムの構築が必要であろう。また、事業システムの中に評価を位置づけるためにも、評価指標として活用できる報告書のあり方を検討する必要性が明らかになった。4)今後は、調整推進事業における地域のケア体制づくりの形成過程を、帰納的に分析するとともに、オンタイムな質的分析を行いたい。

The necessity of evaluating the position of the enterprise and the use of the report is clear. 4)In the future, we will adjust and promote the process of formation of regional and institutional systems, and analyze the quality of the system.
