Helicobacter pylori (H. pylori)は遺伝学的に多様性のある菌種である。遺伝学的に分類したH. pyloriのタイプ別の除菌の是非を明らかにする目的で、H. pyloriのタイプと疾患特異性およびウレアーゼ活性について検討した。対象はH. pylori陽性の胃潰瘍25例、十二指腸潰瘍16例、萎縮性胃炎7例の計48例である。H. pyloriの分類はPCR法を用い、プライマーはUreCを使用し、Acc2、Hha1、Mbo1消化を行った。また^<13>C-尿素呼気試験を用い^<13>Cを標識した尿素を内服させ、内服前と内服後20分の^<13>C_2値の差をウレアーゼ活性と定義した。疾患別のウレアーゼ活性は、十二指腸潰瘍、胃潰瘍、萎縮性胃炎の順に高値を示し、十二指腸潰瘍と萎縮性胃炎の間に有意差を認めた。前庭部および体部の菌量とウレアーゼ活性との間には相関関係は認められなかった。H. pyloriは各制限酵素消化で5〜10タイプに分類された。今回の検討では遺伝学的に分類したタイプが多岐にわたり、タイプと疾患およびウレアーゼ活性の間には明らかな関係を見出し得なかった。しかし疾患によりウレアーゼ活性に有意の差がみられるにも関わらず、ウレアーゼ活性は菌量と相関しないことより菌種の違いによるウレアーゼ活性の違いが示唆された。したがってこの検討は今後も進めて行く価値があると考えられる。今回はUreCをターゲットにしたが、ウレアーゼ遺伝子は他にも存在する。また空胞化毒素などのサイトトキシンとの関連も注目されている。向後症例数を増やし、プライマーを変化させ遺伝学的タイプと疾患特異性について追求する予定である。またウレアーゼだけでなくサイトトキシンの観点からも検討を加えてゆきたい。近い将来消化性潰瘍のみならず胃癌との関連でH. pyloriの除菌が問題となってくると考えられるが、本邦におけるH. pylori陽性者数を考慮すると、全例除菌することは適切ではない。H. pyloriの遺伝学的多様性と疾患特異性の検討を進めることは、除菌の是非を検討する上で意義があると考えられる。
