1.血中抗基底膜抗体を有する自己免疫性水疱性患者として知られている水疱性類天疱瘡、瘤痕性類天疱瘡、後天性表皮水疱症、線状IgA水疱性皮膚症とは臨床後、免疫生化学的特徴の異なる水疱症患者5例の血清について解析を行った。この5例は、病変部皮膚においてIgG及び補体の基底膜部への線状沈着を認め、通常のヒト皮膚を用いた蛍光抗体間接法にてIgGクラスの抗基底膜抗体を認め、これは1M食塩水剥離皮膚による蛍光抗体間接法では真皮側と反応するが、前述の既知疾患とは異なり免疫ブロット法にて真皮抽出物中の200kD蛋白と反応するIgGを有することが判明している。この200kD蛋白の性状を解析するため、以下のような検索を行いそれぞれ結果を得た。2.真皮抽出物中の200kD蛋白と反応するIgGをaffinity purificationにより抽出、濃縮し1M食塩水剥離皮膚による蛍光抗体間接法を施行したところ、血清同様真皮側と反応した。従ってこの200kD蛋白と反応するIgGは血中の抗基底膜抗体であり、非特異的な反応ではないと考えられた。3.その分子量及び皮膚での局在から、ラミニン1β鎖との異同を確認するため、ヒト胎盤由来のラミニンを用いた免疫ブロット法及びマウスのEHS tumor 細胞培養液抽出物を抗原源とし銀発色による免疫沈降法を施行したが、全症例に反応を認めなかった。4.他国共同研究者により患者血清とラミニン5、6との反応を免疫沈降法により検討されたが、いずれとも反応は認めなかった。5.培養正常ヒト表皮細胞及び線維芽細胞抽出物を抗原源とし、免疫ブロット法、免疫沈降法を施行、200kD蛋白の分泌細胞を検索中である。6.以上のように、この200kD蛋白に既知の基底膜部蛋白で相当するものはなく、今後は患者血清と各種培養細胞由来のcDNAライブラリーを用いた抗原蛋白のクローニングなどを行う予定である。

1. Anti-basement membrane antibodies were detected in the serum of 5 patients with vesicular pemphigoid, nodular pemphigoid, acquired epidermal vesicular disease, linear IgA vesicular dermatosis, and post-clinical, immunobiochemical features of vesicular disease. The 5 cases were identified by linear deposition of IgG and complement in the basement membrane of the diseased skin, and the normal skin was identified by indirect method of IgG and anti-basement membrane antibody. The 5 cases were identified by indirect method of IgG and complement in the dermis. The above known diseases are different from those identified by immunologic methods, such as 200kD protein and IgG in dermal extracts. The analysis of the 200kD protein traits was carried out in the following ways: 2. The 200kD protein in the dermal extract was purified by affinity purification, extraction, concentration, and 1M dietary water. The 200kD protein in the IgG antibody in the blood, non-specific antibody in the test 3. The molecular weight and the structure of the placenta were determined by immunoblot method and immunoprecipitation method. 4. Other co-investigators examined the serum levels of the patients and found that the serum levels of the patients were higher than those of the controls. 5. Cultured normal epidermal cells and germ cell extracts were used as antigen sources, immunoblotting methods, immunosedimentation methods, and 200kD protein secreting cells. 6. The 200kD protein is known to correspond to the basement membrane protein, and it is expected to be used in the future for the generation of cDNA from various cultured cells in patient serum.
