直腸切断術後に本来外肛門括約筋を支配していた陰部神経を神経縫合した骨格筋を使用して肛門括約筋を再建し新肛門を造設する術式の試みの実現性を,犬での実験とヒトの解剖体における計測で検討をした。【犬での実験】(米国学術誌Surgeryに掲載予定)外肛門括約筋を含めて直腸肛門部を切除した後,陰部神経縫合群では陰部神経を支配神経に縫合した大腿二頭筋を旧肛門部に有茎移植することにより括約筋を再建し,会陰部に新肛門を形成した。術後にdefecography,manometry及び筋電図検査を施行し,採取した筋肉にactomyosin ATPase染色を施し組織化学的検討を行った。陰部神経縫合群で安静時肛門直角角と肛門管長でそれぞれに有意差を認め,対照群で肛門直腸角で安静時と括約筋収縮時との間にのみ有意差を認めた。肛門管へのmicroballoon挿入時の一過性の内圧上昇反射が陰部神経縫合群で69%に認められた。また安静時筋電図で他の骨格筋や対照群の筋と異なり基礎波が観察でき,肛門管へのmicroballoon挿入時に反射的な筋電図波形の増幅が観察された。有意に陰部神経縫合群の再建括約筋が,その筋繊維タイプを大腿二頭筋から外肛門括約筋に変化していた。陰部神経と神経縫合した部位より移植筋側の神経の断面では多数の再生した有髄神経繊維が観察された。陰部神経縫合群で良好な排便状況を示した。【解剖体における検討】解剖体を使用して陰部神経を神経縫合した下部大臀筋を用いた再建括約筋による会陰部の人工肛門を造設する手術を施行し,解剖学的な問題点について検討した。ホルマリン固定後アルコール置換された教育用献体を使用し,右下臀神経と右陰部神経の分枝状態を観察及び計測し,また実際に解剖体で下部大臀筋による会陰部人工肛門を造設したところ,解剖学的に十分可能な術式であることが明らかとなった。【結論】陰部神経縫合法を用いた機能的会陰部人工肛門造設術によって直腸切断術後に再び新肛門が形成できる可能性が示唆された。

Intended to re-establish the genital nerve suture group, the muscle tendon, the thigh biceps tendon and the external anal tendon. The nerve suture site of the pudendal nerve is located on the side of the graft tendon, and most of the nerve regeneration is observed. The vaginal canal suture group is in good defecation condition. Anatomical examination Anatomical examination After fixation, the artificial anus of perineum was constructed by observing and measuring the branching state of right lower buttock and right pudendal nerve, and the anatomical formula was very possible. [Conclusion] The possibility of new anus formation after rectotomy is demonstrated by the use of perineal neurorrhaphy.
