1)臨床例についての検討:直腸癌神経温存手術男子患者30例について誘発球海綿体反射測定(後部尿道刺激、陰茎刺激)と尿流動態検査を手術前、術語1-3ケ月、6ケ月に行いその結果を検討した。術前の測定では陰茎刺激で知覚閾値1.9±0.6mA潜時33.3±4.2msec、後部尿道刺激で、視覚閾値5.6±1.8mA潜時65.0±9.3msecであった。術後早期(1-3カ月)に尿流動態検査で膀胱機能が正常の例では、術前の測定値と有意差がなかったが、膀胱機能に異常を呈した例では後部尿道刺激での知覚閾値が有意に上昇し、4例で後部尿道刺激の反射が消失していた。個々の症例で検討しても後部尿道刺激の知覚閾値の上昇と膀胱機能の異常とは有意に相関した。また、膀胱機能に異常を呈した11例のについて術後早期の後部尿道刺激の知覚閾値の上昇の有無と術後6カ月までの膀胱機能の回復とは有意に相関した。また後部尿道刺激の反射が消失していた4例ではいずれも膀胱機能は正常化しなかった。後部尿道刺激による誘発球海綿体反射測定は直腸癌術後の膀胱機能障害の回復の予測をする上で有用と考えられた。2)実験モデルによる検討:吸入麻酔下および除脳イヌを用いて内尿道口から5cmまでの尿道と膀胱頚部・膀胱体部(後壁)を刺激し肛門括約筋から導出する誘発球海綿体反射について検討した。すべての部位で陰茎刺激と同様の波形・潜時の反射が記録され、この反射は骨盤神経の切断前、片側切断、両側切断時において変化がなかったため、陰部神経を求心路とする反射と考えられた。イヌでは後部尿道から膀胱頚部と膀胱体部の一部まで陰部神経の視覚枝が分布していることが推察されたが、ヒトでみられる骨盤神経を介する反射は記録されなかった。

1)Clinical study: 30 male patients undergoing neurosurgery for rectal cancer were examined for the results of the study, including the measurement of bulbocavernous reflexes (posterior urethral stimulation, penile stimulation) and urine flow patterns before surgery, 1-3 months and 6 months. Before operation, the threshold value of penile stimulation was 1.9±0.6mA, latency was 33.3±4.2msec, and the threshold value of posterior urethral stimulation was 5.6±1.8mA, latency was 65.0±9.3msec. Early postoperative urine flow patterns revealed normal bladder function in 1-3 months, preoperative measurements in 1 - 3 months, abnormal bladder function in 1 - 3 months, posterior urethral stimulation in 1 - 3 months, and disappearance of posterior urethral stimulation in 4 cases. The symptoms of bladder dysfunction are discussed in detail below. 11 cases showed abnormal bladder function, and there was no significant correlation between early postoperative posterior urethral stimulation and the increase of bladder function threshold, and 6 months after operation. The posterior urethral stimulation reflex disappeared in 4 cases and the bladder function normalized. Posterior urethral stimulation induced glomerular cavernous reflex measurement is useful for predicting recovery of bladder dysfunction after rectal cancer surgery 2) Inhalation of anesthesia and removal of anesthesia from the urethra to the urinary bladder (posterior wall) may stimulate the anal muscles and induce the globocavernous reflex. The penile stimulation and the latency reflex are recorded before, during and after the pelvic nerve is cut. The posterior urethra, the bladder, the body of the urinary bladder, the distribution of the optic branches of the pudendal nerve, and the reflection of the pelvic nerve are recorded.
