説明し同意を得た産婦が分娩第I期,第II期にあるとき,B・Mモード法,カラードプラ法及びパルスドプラ法を備えた超音波検査装置により,陣痛発作時・間歇期の臍帯動脈血流速度波形を可及的に連続して計測し記録した.胎児徐脈のない場合,陣痛発作時と間歇字の臍帯動脈血流速度波形のresistance index (UmA-RI)の平均値には有意差がみられなかった.次いで一過性胎児徐脈を認める場合,徐脈の前後でUmA-RIを比較すると,早発一過性徐脈では徐脈の前後でその平均値に有意差がなかった.これに対し変動一過性徐脈では,徐脈発生時のUmA-RIは高値をとる場合が多く,その平均値は徐脈発生前のUmA-RIの平均値に比し有意に高値となった.一般的には早発一過性徐脈の原因は児頭圧迫による神経反射で,変動一過性徐脈の原因は臍帯圧迫によるものと考えられているが,この結果はそれらとよく合致する.次いで,変動一過性徐脈の発生前・中・後の胎児心拍数の変化とUmA-RIの変化との関係を検討するため,変動一過性徐脈を1.発生前,2.児心拍数加工期,3.再徐脈期,4.回復期,5.回復直後の5期に分けた.それぞれの期のUmA-RIの平均値は1.期,4.期に比し3.期が有意に高値であるだけでなく,2.期と4.期は児心拍数の平均値には有意差がないのに,UmA-RIは4.期に比し2.期が有意に高かった.このことは,臍帯圧迫が発生しUmA-RIが上昇することに,神経反射である徐脈の発生がやや遅れて起こり,さらに臍帯圧迫が解除されてUmA-RIは既に元の値に復する時に,徐脈の回復がやや遅れることを示しているものと考えた.このようにパルスドプラ法によるUmA-RIの観察は,分娩時に胎児仮死の主要な原因とされている臍帯圧迫の存在を検出する良い手段になりうるものと考えられた.
