本年度は側頭筋皮弁充填群における嚢胞形成抑制効果をコントロール群と比較検討した。1.実験方法実験には鼓膜に異常所見のない10匹のモルモット(350-700g)を使用した。既に報告した耳介皮膚の中耳背側骨胞への遊離移植により、移植3週後に嚢胞の形成を確認した。これら嚢胞壁の一部を破り内部の表皮剥屑物を可及的に掻き出して嚢胞周囲の肉芽組織の中に散布した。以後の操作により動物を2群に分けた。(1)充填群:有茎の側頭筋皮弁を用いて中耳腔を完全に充填した。出血のないことを確認し創を縫合閉鎖した。(2)コントロール群:充填は行わず、そのまま創を縫合閉鎖した。動物は2週後、4週後、8週後に致死させ中耳を摘出した。4%ホルマリン固定の後EDTAを用いて脱灰し、パラフィン包埋後に連続切片を作成してHE染色を施し、組織学的検討を行う予定である。2.実験結果と考察標本摘出時に手術用顕微鏡を用いて嚢胞の再形成の有無を観察した。充填群においてもコントロール群同様、全例で真珠腫の形成が認められた。充填群の嚢胞はコントロール群より最大径が大きい傾向が認められた。組織学的検討はまだ行っていないので詳細は不明であるが、側頭筋皮弁から嚢胞に良好な血流が供給されたことにより、嚢胞の成長が促進された可能性が考えられた。本実験により、側頭筋皮弁により中耳腔の充填では嚢胞の形成抑制効果は得られない可能性が示唆された。
