活動的平均余命算出のためのStandardizationを確立するために、サンプリング調査をおこなった。対象地域は沖縄本島南部市町村に属する玉城村で、対象者は同村に過去20年間居住した85歳以上高齢者135人とした。調査は高齢者宅を戸別に訪問し、1)生活背景2)生活自立度(ADL)3)知的能力(痴呆度)の3側面からおこなった。生活自立度に関しては厚生省判定基準に基づき、知的能力に関しては、知能評価スケール(HDS-R)を用いた。その結果、ADLスコアは、男性平均49.58点、女性46.06点で男性は女性に比べ高かった。ADL項目別でみると、視・聴覚などの感覚機能は食事、排泄などの身体活動能力や意思表示、会話の理解などの認識能力に比べ劣り、特に聴力はスコア4ポイント以下であった。寝たきり度は、生活自立(J)が約8割、準寝たきり(A)が約1割で、残る1割が寝たきり(B,C)であった。HDS-R検査では、「痴呆症状無」が男性6割で、逆に「痴呆症状有」が女性6割であった。HDS-R10点以下の「強度の痴呆」は対象者の15.7%を占めていた。生活背景調査に関しては、職業歴、学歴、結婚歴、嗜好、性格等の多方面からの聴取を行った。職業歴では農漁業従事者が大半を占めていた。また、最終学歴は尋常小学校卒業が63.1%と半数以上を占めていた。玉城村の20年前の65歳以上のコホート群876名中の20年度の現在の生存率は274名31.3%であったが、厚生省寝たきり者率のB,C群とHDS-R10点以下を除いて、それらから重複者を除いた結果得られたアクティブ生存率は257名29.3%となった。それは1996年に行った結果に比較すると7.5%上回っていた。1995年の小椋らの行った沖縄県の痴呆性老人並びに1996年に我々の行ったDisability老人に関する全県調査は地域及び対象をサンプリングして行ったものである。従って、これらの調査は地域差が大きいので地域をきめ細かに、且つ悉皆的に調査しなければならないことがわかった。
