RI標識白血球の骨髄への集積が真に白血球造成能を反映するか否か、放射線療法や化学療法等により骨髄機能抑制が疑われた悪性腫瘍患者群を対象として骨髄予備能の検討を行い、得られた結果から悪性腫瘍患者における放射線療法や化学療法による治療成績向上への応用を図ると共に、白血球数の減少が見られた患者におけるサイトカイン投与の適応性の有無を決定することを研究主目的とする。また同時に、より標識操作が簡便な白血球標識法を確立することをも目的とする。上記の研究目的に従い、平成8年度中に得られた研究成果のまとめは、下記の如くである。1.RI標識白血球の骨髄集積機序の解明に関する検討。放射線療法や化学療法施行中の悪性腫瘍患者群で施行されたRI標識白血球イメージング所見は、骨髄穿刺所見との対比より、明らかに白血球造成能を反映し、骨髄予備能を示す結果が得られた。2.放射線療法や化学療法中に白血球減少が見られサイトカインを投与した患者群での検討。これらの患者群では、サイトカイン投与により末梢血液中の白血球数に改善が見られても、必ずしも骨髄予備能改善に寄与しないことが、RI標識白血球イメージング所見の結果より示された。以上、平成8年度に得られた結果をもとに、悪性腫瘍患者の更なる治療成績向上とサイトカイン投与是非の決定に関する検討を継続する予定である。なお簡便な白血球標識に関しては、未だ満足すべき十分な結果が得られなかったが、標識率の改善を目指して引き続き検討を加えていく予定である。
