外科治療に伴う周術期に肺微小血管透過性亢進が病態の本体であると考えられる成人呼吸促迫症候群(ARDS)を発症する事があり、その治療が困難であるため致命的な合併症のひとつとしてその予防法の確立が急がれている。その発生機序として多核白血球がサイトカインやアナフィラトキシンにより活性化され肺血管障害を来す事が注目されてきた。研究代表者は補体活性経路の最終産物である可溶性後期複合体(SC5b-9)が肺微小血管の透過性亢進を来す知見を得て、これが微小血管内皮細胞上のインテグリン受容体を介する細胞内シグナル伝達によるものあることを発表してきた。ヒト生体で外科手術周術期に、血中SC5b-9がどの程度変化するものか、動物実験で示された肺血管障害を来す濃度にまで達するのかを、肺に直接手術操作のおよぶ肺切除例で検討した。患者血漿中のSC5b-9濃度をELISAを用いて定量した。肺葉切除後に肺血管抵抗値の上昇(平均肺動脈圧40mmHg)を来した例では術後1日め、2日めに3.1、3.3μg/mlと高値を示した。通常の術後経過をとった肺葉切除、肺部分切除では冠動脈バイパス術を同時に行った例でも0.2〜0.3μg/mlと上昇は認められなかった。肺全摘では、術後1日め、3,7、3.5μg/ml2日めに2.4、1.6μg/mlと高値であった。胸膜肺摘除の1例では、術直後、術翌日は上昇を認めなかったが、2日めに3.7μg/mlと上昇した。また肺部分切除例でも複数箇所に手術操作の及んだものでは1.2μg/mlと軽度上昇を示した。手術侵襲の程度に応じ後期補体成分形成の亢進する可能性が示唆される。しかしながら、動物実験で示した肺血管障害を来す濃度に比してその濃度上昇は軽度であった。この臨床結果からは、SC5b-9が更に形成される様な感染等の併存や、肺静脈圧上昇などが加わらないと肺微小血管透過性亢進は起こらないと考えられる。
