平成9年5月より12月までに、本研究の主旨の了解を得られた在宅療育中の病障害児13名およびその家族29名(母親13名、父親5名、同胞11名)、計42名に対し、平成8年度の研究分析結果を踏まえて改良した看護実践モデルの試行を行なった。実践には、本研究メンバーの他、小児臨床看護婦2名および基礎看護技術を既に習得した看護学生55名の協力を得た。改良点としては、面談やディスカッションの場を意識的に増やし、プレイ計画の立案から評価まで、看護サービスの需要者である対象者と提供者である看護者とが共同して行なう、インフォームドコンセントを重視した看護展開を行なった。尚、プレイの実施回数や形態、組織構成などに関しては平成8年度とほぼ同様である。その結果、前年度以上の成果が検査や観察結果に認められた。今回、母親がプレイ計画の段階から関わったことで、母親は対象児の抱える問題点の明確化、日常生活での適切な役割行動、家庭内外での療育姿勢の一貫性を得ることが出来たと考える。これにより、母親は前向きな情緒の安定、実感的自信を得て、発動性を発揮し、母親としての自己実現を果し、子どもにも良い影響が及んだものと考える。2年間の最終成果として、プレイにおける援助モデルとその臨床実践方法が開発出来たと考える。なお、この研究プロジェクトは、対象児の母親らの賞賛と強い要望を受け、研究終了後も継続実施することになった。このことは臨床実践として発展の可能性の高いモデル開発がなされた結果と考えている。今後も看護サービスの需要者(母親ら)の意見を検討しながら、小児看護(主に発達援助)および家族看護(主にメンタルケア)の可能性を追求する場として発展させて行きたいと考えている。
