项目摘要

ホジキン病、非ホジキンリンパ腫合併例において、組織切片よりmicromanipulatorを用い一個の腫瘍細胞を採取し、single cell PCR法を用い免疫グロブリンH鎖遺伝子を増幅することにより、両細胞クローンの関係を検討した。従来、ホジキン病と非ホジキンリンパ腫とは互いに異なった関連性のない腫瘍と考えられていたが、single cell PCR法を用いた最近の研究からは、ホジキン病の腫瘍細胞はB細胞由来であることが示されており、あらためて両腫瘍細胞の関係を検討する必要が求められている。今回我々は、三つのタイプの合併例{(1)慢性リンパ性白血病とホジキン病、(2)結節性リンパ球優勢型ホジキン病と大細胞型リンパ腫、(3)ホジキン病治療後に発生した非ホジキンリンパ腫}において、両腫瘍細胞の関係を上記方法を用いることにより検討した。B細胞型慢性リンパ性白血病患者におけるホジキン病合併例の検討では、3例中2例において、両腫瘍細胞クローンが同一であり、ホジキン病が慢性リンパ性白血病からの進展腫瘍であることを明らかにした。また結節性リンパ球優勢型ホジキン病患者におけるB細胞型大細胞型リンパ腫合併例2例における検討においても、同様の結果を得た。一方、結節硬化型ホジキン病の治療後に発生したB細胞型非ホジキンリンパ腫の1例における検討では、両腫瘍細胞は別々のクローンに属することを明らかにした。以上、これらの結果から、ホジキン病が単一のクローン性B細胞増殖疾患であることが示されるとともに、ある条件下においては両疾患が相互に移行し得ることが明らかとなった。

To investigate the relationship between tumor cells and the expression of tumor cells, a single cell PCR method was used to detect the expression of tumor cells. Recent studies have shown that the relationship between tumor cell and B cell is essential. This paper reviews the combined cases of chronic leukemia, nodular leukemia and macrocytic leukemia, and the development of leukemia after treatment. A review of B-cell type chronic leukemia patients with concurrent disease was conducted in 2 out of 3 patients with B-cell type chronic leukemia. The results of the study were similar in 2 cases of nodular polycystic disease and 2 cases of B cell polycystic disease One case of tuberous sclerosis developed after treatment, and one case of B-cell non-tuberous sclerosis developed after treatment. The above results show that B cell proliferation is a common disease.
