平成10年度から12年度にかけて42個(42例)の頚部リンパ節病変(n=29)と甲状腺(n=12)、顎下腺病変(n=1)に超音波誘導下吸引細胞を施行し、検体のDNA遺伝子再構成の有無を検討した。病理診断の内訳はB細胞悪性リンパ腫(初発18例、再発6例)24病変、ホジキン病1病変、上皮性癌5病変、反応性リンパ節6病変、橋本病の甲状腺組織6病変である。これらの内、遺伝子再構成陽性と診断されたのは13病変であった。真陽性は10病変で偽陽性3病変(反応性リンパ節、橋本病、甲状腺乳頭癌)、真陰性15病変、偽陰性14病変であった。結果として、当検査法の有病正診率は42%、無病正診率83%、正診率60%となった。当検査法の有病正診率は、組織を用いた遺伝子再構成の有無によるB細胞悪性リンパ腫の診断能と比べると劣っていたが、これが技術上の問題であるのか吸引細胞での採取細胞数の少なさに起因するのかについては、今後さらに検討していきたいと考えている。偽陽性の原因についても症例を積み重ねて検討していく予定である。

From 2000 to 2001, there were 42 cases (42 cases) of thyroid diseases (n=29), thyroid diseases (n=12), submandibular diseases (n=1), ultrasound-induced suction of cells, and the presence or absence of DNA gene reconstruction in the body. Pathological diagnosis included 24 cases of internal B cell lymphoma (initial 18 cases, recurrent 6 cases), 1 case of thyroid cancer, 5 cases of epithelial carcinoma, 6 cases of reverse thyroid cancer, and 6 cases of thyroid tissue. The positive diagnosis was made up of the internal and external organs. True positive 10 diseases, false positive 3 diseases (anti-thyroid disease, Hashimoto disease, thyroid papillary carcinoma), true negative 15 diseases, false negative 14 diseases. Results: The positive rate of disease was 42%, the positive rate of no disease was 83%, and the positive rate of diagnosis was 60%. When the diagnostic method has a positive diagnosis rate of disease, tissue use, gene recombination and the presence or absence of B cell sex, the diagnostic ability of the tumor is inferior, and the technical problems are caused by the lack of cells to attract cells and the number of cells to be taken. False positive causes include multiple cases, multiple cases, and multiple cases.
