本研究の目的は,看護婦が臨床における道徳的問題を認知する契機をつくる「道徳的感受性」を涵養するための研修を実施することを前提に,その教育効果を測定する質問紙を作成することである。特に,拘束や老人虐待などの問題を指摘されている老人看護の領域で働く看護婦の道徳的感受性を測定する質問紙の作成を目指している。これまでの成果として,20人の看護婦に面接した結果,臨床で看護婦が経験している問題は,ジレンマの形で表現され,それは以下のようなカテゴリーに分類された。患者の権利などの倫理的原則に関するもの,人間関係についての認識,職業人としての自負心,医学と看護学との価値観の違いに基づく葛藤,よさへの志向,施設の方針・システムとの葛藤。特に,老人看護の領域における特徴とみられることは,施設が看護者の人数を十分に雇用しないために生じる「忙しさ」などのようなかたちで現れてくる施設の方針あるいはシステム上の問題,「高齢者への差別意識」などの具体的な現れである「拘束しておけばいい」というケアの方針などを変更させることができない「人間関係」などにおける問題として分類された。文献上に見られる「社会的公正」に関する葛藤状況は,抽出されていないために飽和に達しているとは推論できないために,さらに,データ収集及び分析が必要である。面接法の限界として,被面接者が意識していないことは表現されないということがある。今後は,面接よりは,文献の分析に重点をおき,継続して質問紙の作成を試みたいと考えている。
