项目摘要

Chlamydia trachomatis感染症は性感染症の中でももっとも頻度が高いものの一つであり、近年、特に若年者において蔓延していることが問題となっている。また、低用量ピルが解禁され、その影響も懸念されている。海外からの研究論文によって、その感染と関連がある因子がいくつか報告されているが、わが国では感染の危険因子に関する詳細な研究報告は、これまでにほとんどなかった。そこで、我々は、長崎県における妊婦のChlamydia trachomatis抗体スクリーニング対象者に対し、抗体陽性に関連する社会的属性や妊娠歴等の影響の検討をおこない、以下の結果を得た。1.抗体陽性率は20.8%であった。2.多重ロジスティック解析の結果、高い抗体陽性率と関連があったのは、(1)結婚前の妊娠歴、(2)コンドームの不使用、(3)短い教育歴、(4)より多い人工妊娠中絶回数、の4項目であった。本研究では、低用量ピルに対する若年者の意識と性行動・性意識との関連を知るために平成11年度から13年度にかけて、大学生を対象とした調査を行った。その結果、1.性交経験などの性行動に変化はなかった。2.ピル使用に関しては賛成という意見が多かったが自分自身の使用に関しては否定的な考えが多かった。3.ピルと性感染症やエイズとの関係に関しては、一般に行われている調査と比べて正答率が高かった。4.コンドームの使用をパートナーが決めるという女性が約半数であり、女性が自分で避妊法を決定できるという意味で、ピルの重要性が改めて示唆された。ピル解禁における影響を追跡するために本研究は今後も継続予定である。
