正常群2,040例における母体血清log AFPをy軸に,妊娠日をx軸に設定しグラフを作製すると,回帰直線はy=0.0093x+0.6(r=0.430,p<0.001)となった.また正常群2,054例における母体血清AFP-L3%をy軸に,妊娠日をx軸に設定しグラフを作製すると,回帰直線はy=-0.1321x+41.77(r=-0.147,p<0.001)となった.これらの回帰直線から算出した正常群のAFP MoMの中央値は1.0(範囲;0.6-1.5),またAFP-L3%MoMの中央値は1.0(範囲;0.0-2.4)で,21トリソミーにおけるAFP MoMの中央値は0.9(0.3-1.2),またAFP-L3% MoMの中央値は1.7(1.1-2.1)であり,AFP MoMにおいては,21トリソミー群は正常群に比較し有意(p=0.0003)に低値を示し,AFP-L3% MoMにおいては,21トリソミー群は正常群こ比較し有意(p<0.0001)に高値を示した.ROC曲線を用いた21トリソミーの検出法としての有用性の検討では,AFP MoMとAFP-L3% MoMのAUCはそれぞれ0.709と0.937であり,AFP-L3% MoMのAUCが有意(p<0.000024)に高値を示した.またそれぞれの偽陽性率を5%に固定すると,AFP MoMのカットオフは0.85>となり感度は24%,またAFP-L3% MoMのカットオフは1.50<となり感度は66%であった.AFP-L3% MoM,AFP MoM,hCG MoM,uE3 MoMの4種の母体血清マーカーを組み合わせスコア化により検討すると,21トリソミーの診断における感度は85.2%,偽陽性率は3.0%であった.またこの場合のpositive predictive valueは65.8%であった.以上の結果,レクチンと抗AFPモノクローナル抗体の競合反応を応用したliquid-phase binding assayを用いて正常妊娠婦人の母体血清AFP-L3%を測定することにより,個々のAFP-L3%をMoM値で評価することが可能となった.AFP-L3% MoMは21トリソミーの出生前診断において,単一の母体血清マーカーとしても診断効率の上昇をもたらすことが初めて証明され,またAFP-L3% MoMを他の母体血清マーカーと組み合わせることで,従来のトリプルマーカーテストに比較し著明な感度の改善が得られることが示唆された.

The AUC of AFP MoM and AFP-L3% MoM was found to be 0.709 and 0.937 respectively. The AUC of AFP-L3 % MoM was found to have a high value (p<0.00024). The false positive rate of AFP MoM was 0.85%, the sensitivity was 24%, the sensitivity was 1.50%, and the sensitivity was 66%.AFP-L3 % MoM,AFP MoM,hCG MoM,uE3 MoM were used to detect the false positive rate of AFP MoM, and the sensitivity was 85.2%. False positive rate 3.0%. The positive predictive value of the field combination is 65.8%. These results suggest that the anti-AFP antibody anti-AFP antibody anti AFP-L3% MoM is the most common form of maternal serum.
