臨床的NO口腔癌に対し99mTcフチン酸を用いてセンチネルリンパ節同定を行い、同リンパ節と他の頸部リンパ節における癌転移の有無につき検討した。(対象)舌癌10例、口腔底癌3例、頬粘膜癌、下歯肉癌それぞれ1例。頬粘膜癌のみ粘表皮癌で、他は全て扁平上皮癌であった。(方法)手術前日に先の10例には37MBqを、後の5例には18.5MBqを原発腫瘍周囲粘膜下に注射した。手術開始前にガンマプローブを用いて経皮的に放射能活性のあるリンパ節部位を検索した後、術野で、頸部リンパ節郭清に先立って放射能活性のあるリンパ節をセンチネルリンパ節として先に摘出した。その後通例どおり手術施行し、郭清標本中で再度ガンマプローブを用いてセンチネルリンパ節を同定した。それぞれのセンチネルリンパ節、郭清リンパ節につき病理組織学的に検索した。(結果)15例全てにおいて1個以上のセンチネルリンパ節が同定された。5例6個のセンチネルリンパ節に転移が認められた。2個は明らかな転移で他の4個は微小転移であった。他のリンパ節には転移を認めなかった。転移が認められたリンパ節の放射能活性は最も高いか、2番目または3番目であった。摘出標本から見出されたリンパ節の放射能活性は総じて低かったが1例においてLevelIより最も放射能活性の高いリンパ節が見出された。(考察)センチネルリンパ節における癌転移の有無が個々の症例における頸部リンパ節転移の状態を反映すると考えられた。放射能活性の高いリンパ節から順に3個程度を摘出すれば転移の有無を予測できると考えられたが、摘出標本のLevelIから放射能活性が最も高いリンパ節が見出された症例がありLevelIでの同定では原発巣からの放射能活性のオーバーラップに注意すべきであると考えられた。
