【方法】1.対象:入院後1ヶ月以上経過した65歳以上高齢者のうち調査協力への同意が得られ、コミュニケーションが可能な者17名(男性6名、女性11名)であり、入院前の世帯構成は、独居が8名、高齢者二人世帯が9名であった。2.調査方法および分析:半構成的な質問紙を用い、30〜50分の面接調査を行なった。了承が得られた者については面接内容を録音し、それ以外の者ではメモをとり、面接終了後本人の言葉に基づいて記録したものを整理分析した。【結果と考察】1.退院先の希望:本人の希望する退院先は、介護の必要性に関わらず「自宅」であった。2.実際の退院先:高齢者二人世帯においては、本人、同居者ともに高齢であっても自宅退院したのに対し、独居者については要介護度が高い者は病院または施設への退院となり、健康度や自立度が退院先に大きく影響していた。3.入院前の社会的ネットワーク:どの対象も何らかの社会的ネットワークを有し、そこからのサポート(情緒的・手段的サポート)を得ていた。独居者では自宅退院を希望する背景として、親族が同敷地内に居住している、なじみの酒屋が単品でも配達してくれる等身近な手段的サポートの存在への認識がうかがえた。支援の提供者としては、同胞や同年齢の友人は高齢という理由から電話などを通じての情緒的な面での支援者となっており、子供や甥姪の他、商店を含む近隣やヘルパーが手段的な面での支援者であった。独居高齢者が自宅退院するにあたっては、身近な具体的な支援をいかに得るかが重要である。また、緊急事態に対する別居の親族の懸念により独居高齢者が自宅退院をあきらめており、たとえ同居者が高齢であっても他の誰かの存在は大きいといえることから、緊急通報システム等のハード面に加え、見守り等も含めた近隣のネットワークや地縁づくりへの行政等によるサポートが求められるのではないか。
