项目摘要

本年度の研究では、まず、医療社会学の巨人・パーソンズの再検討をつうじて、彼が論じなかった病人役割/患者役割の区別、および医療内部における緊張関係を抽出した。パーソンズはそれらに注意を払わなかったがゆえに、医療を非常に静態的なものと理解したのだった。むしろ近代医療システムは、病人と同定と患者の同定の2段階的な構成をとっており、また<治療-教育-学問>という内的緊張を孕むがゆえに、動態的なものとなるのである。つぎに、本研究者は、イリッチからコンラッド＆シュナイダーに至る医療化概念を「境界再設定過程」と捉え返し、医療システムの動態を把握するための枠組みを用意した。医療システムとはケア・コミュニケーションを要素とする構造の過程であり、それが学問や教育との特殊な関係を取り結び、かつ社会全体の分業体系のなかで特有の位置を占めるときに近代医療システムは成立する。この枠組みでは、従来の「拡大過程としての医療化」とともに、その反対物と見なされた脱医療化過程をも同時に理解できる。また、その動態性をもたらすものは、医療内部に由来する医学的知の蓄積への要求と、医療の外部に由来する「生権力」の水準差である、と論じた。この枠組みで臓器移植という現象を理解すると、第1に、死についての医学的知の徹底化があり、また医療システムの自己準拠化の徹底としての延命至上命令の隘路として把握することができる。だが、こうした医療化が困難につきあたったために、第2に、治療停止という脱医療化、そして社会政策の導入といういみでの脱医療化、さらには臓器移植を首尾よく達成するための精神的ケア-物語論の導入など、直線的な拡大過程ではない「医療の境界再設定過程」が同時に確認されるのである。臓器移植では、一方では自己閉塞的な医療の限界、他方では拡散ゆえの医療の独自性の喪失(医療の経済現象化)という2重の危機が医療を見舞っている。そしてこの危機を乗り越えるために導入された<社会的身体>という概念が、医療のつぎなるステージを切り開くことになるだろう。臓器移植はそうした痕跡を残してやがて消失する、過渡的な事件だったというのが本研究の結論である。
