発話の明瞭さの指標とともに,明瞭度を低下させている構音の誤りに関する情報まで提供しうる構音障害用明瞭度評価法を作成し,既存の単語明瞭度検査との関連や検査としての妥当性、信頼性を検討した。この検査は,Kentら(1989)が提案した方法にもとづき日本語用に開発したもので、患者が音読する単語リストと,音読音声を一般健聴者が聴取し,発話された単語を同定する際に用いる聴取用単語リスト,結果分析用の異聴パターン表,集計表から構成されている.聴取用単語リストには,構音障害患者が音読した単語(目標語)および,この目標語との間で,基本的に1母音もしくは1子音が異なる点で対をなす語4語が同じ列にランダムに配置されている.患者の構音の誤りの分析にあたり23の音声学的対立を設定した。検査の信頼性,妥当性の検討のために,dysarthria患者10症例に本検査ならびに伊藤の単語明瞭度検査を施行した.患者の音声資料の聴取者は大学生で,症例ごとに5名ずつ,計50名が聴取を行った.その結果、伊藤の単語明瞭度検査との間に強い相関がみられ、本検査は明瞭度の量的指標として妥当性が高いことが示された。聴取者間の一致度は高く、また聴取者内の再現性も高かった。以上から本検査は臨床的に用いる上で高い信頼性を有すると考えられた。また、明瞭度が同程度の症例間で、構音の誤りの質的比較を行うと、誤りパターンに違いがみられた。これは本検査が明瞭度の量的尺度のみならず、明瞭度を低下させている構音障害の状態についての情報を提供しうることを示している。この誤りパターンの分析は音素的対立をベースにしているため、聴覚心理的評価という主観性の強い評定法を、音響分析的にも補うことができ客観的な判定が可能である。本検査は症例の評価だけでなく、訓練効果の測定と、明瞭度改善要因の分析にも応用できると考えられる。
