当該紀南地域1市3町1村は、保健師20名、助産師6名を擁し、小児に対する医師以外の医療資源として重要な役割を担っているが、縦割り行政の弊害のため、十分に効果を発揮しているとはいえない状況である。今回我々は、医療・保健・福祉業務の集約化の方法の一つとして、現状の保健予算の解析、平成14年8月から同年12月までに保健師日報を基礎とした保健師の時間コストの解析、そして保健師へのインタビューを通して、保健事業合併による事業効率化が与える影響について考察した。1.管内各市町村から平成13年度決算書、平成13年度保健事業等国庫(負担)補助金の事業実績報告、平成13年度母子保健衛生費国庫負担金の事業報告、平成13年度地域保健・老人保健事業報告および紀南地域市町村合併研究会の事務事業実態調査(事業用)から現状の医療・保健状況を分析したところ、人口比の高い老人保健に予算の大部分が使用されていることがわかったが、その多くが委託料となっていた。節減効果を考えると集団健診等の集約化よりも委託料削減(老人基本健康診査を個別から集団へ)の効果の方が大きいことがわかった。2.保健師の時間コストを分析したところ、保健師でなくとも済む業務に全労働時間の4割を費やしていた。現在の各市町村単位の保健師管理を紀南地域全体として集約することで、医療資源を有効活用に寄与できる。また当該地域で一般に弱いとされる健診後のフォローアップに力を注ぐことが可能になる。3.フォーカスグルーディスカッションを基に個々の保健師にインタビューを行ったところ、保健師は住民との接触によってモチベーションを維持していることが確認された。これらの結果から、福祉保健センターの配置としては、末端で地域サービスを行う保健センターとこれを統括する事務局の2つの機能を独立させて保健師管理を行うことが効率的であると思われた。

The time for healthcare professionals to analyze, and the time for healthcare professionals to complete their business At present, the management of health care professionals in each city, town and village is concentrated in the whole region, and medical resources are effectively utilized. When the area is generally weak, the health care system will be able to provide medical care. 3. The health care worker's contact with the residents is maintained. The result is that the health care system is configured to manage the health care system independently.
