本研究では、臨床検体を用いてin vivoにおける子宮頚癌のシスプラチン感受性・耐性規定遺伝子をDNAマイクロアレイにより網羅的に解析・同定し、その遺伝子を用いシスプラチン化学療法を施行すべき子宮頚癌患者の選択基準を確立する事を目的とした。子宮摘出4週間前にシスプラチン(80mg/m2)化学療法を施行した子宮頚癌患者20例を対象とし、MRIによる化学療法施行前後の腫瘍縮小率および摘出標本での組織効果によってシスプラチン有効群またはシスプラチン無効群に分類した。化学療法施行前に得た子宮頚癌組織からのRNAをCy5で、コントロールの正常子宮頚部のRNAをCy3で標識し、蛍光シグナル比率の解析によって、正常組織に比較しそれぞれの癌細胞で増減している遺伝子を検討した。シスプラチン有効群および無効群での遺伝子発現を比較する事により、in vivoにおけるシスプラチン感受性・耐性規定遺伝子を検討した。その結果、薬剤耐性遺伝子、細胞増殖遺伝子、アポトーシス関連遺伝子の幾つかがシスプラチン感受性・耐性と関与しているものと考えられた。今後この遺伝子を利用し、シスプラチン有効患者の選別基準を検討しようと考えている。
