本研究の目的は、在宅要介護者のADL(Ability of Daily Life:日常生活自立度)別にう蝕や要補綴歯の数から歯科治療必要度を推測する地域医療データベースを構築することである。大阪府、兵庫県、和歌山県、奈良県、滋賀県および各府県の歯科医師会の協力を得て、在宅要介護者の日常生活の自立度、要介護度、口腔に関係する生活習慣、硬組織疾患の状態を表す指標、歯科保健指導の必要性など165項目について、604名の被験者を対象とした調査を完了した。調査結果は、第三者が年齢とADLをキーワードとしてインターネットのホームページ経由で検索できるように、専用サーバを準備してそこで公開した。年齢、ADL、人数をキーワードとして検索を行うと、年齢別、性別、ADL別に、現在歯数、未処置歯数、要補綴歯数、咀噛・嚥下指導と訓練の必要性とその機関、歯科保健指導の必要性とその期間がそれぞれ出力される。さらにこれらの変数の中から、未処置歯数、要補綴歯数、歯科保健指導の期間の3つを換算の基準とし、それぞれの値を三次元マトリクスにあてはめて、必要な歯科医師数と歯科衛生士数をのべ日数が換算されて表示される。これらの機能がひとつのWWW用サーバ内で完結するよう開発を行った。歯科治療必要度への換算は、今回用いたように個々の歯科治療必要度を人数分だけ積算してその地域の代表値とする方法の他に、多変量解析法を用いて実際に費やしたマンパワーを目的変数、硬組織疾患や歯科保健指導の期間などを説明変数として算出する方法がある。多変量解析法を用いた方法は、個々の症例にばらつきがあり似たような条件であっても算出されてくるマンパワーの指標が大きく異なってしまうケースが見られたことから、今回は採用しなかった。また、歯科治療必要度にもっとも大きな影響を与える変数は、歯科保健指導の回数と期間であったが、これは地域によって、歯科医師によって判断の基準におおきな偏りがみられた。今後、データベースに蓄積される症例数がさらに増すことによって、歯科治療必要度をより精度高く推定できるものと考えられる。
