若年例の正常眼圧緑内障の原因を解明するために、これまでに報告されているEndothelin-1(ET-1),自己免疫ニューロパチーで抗体価の上昇が報告されている抗ガングリオシド抗体を同時測定し、正常眼圧緑内障の病因としての神経免疫学的機序の関与について検討した。まず、正常眼圧緑内障(NTG)31例(平均年齢49.5才、屈折値-4.6±3.1diopter)、原発開放隅角緑内障(POAG)18例(平均年齢44.7才、屈折値-8.8±3.1diopter)、正常人17例(平均年齢44.5才)に対して視力検査・視野検査・ET-1,ガングリオシド抗体の測定を行った。NTG患者、POAG患者、正常人におけるET-1値はそれぞれ、1.5±1.7pg/ml、1.6±1.8pg/ml、1.4±1.1pg/mlであった。NTG患者とPOAG患者を比較したところ、年齢・視野病期・ET-1に有意な差はなかったが、POAG患者がより近視であった(p<0.001)。抗ガングリオシド抗体は、抗GA1 IgG抗体、抗GQ1b IgG抗体、抗GM1 IgG抗体(+),抗GM2 IgG抗体(+)がそれぞれ1名ずつの異常値を認めたが、いずれも軽度陽性であり、特徴的な視神経所見もなく、特異的なものではないと考えた。全症例のうち上半視野の欠損しているものは8例、下半視野の欠損しているものが12例であった。上半視野症例でのET-1は1.2±0.5pg/mlであった。一方、下半視野症例でのET-1は1.7±0.8pg/mlであり、有意な差はなかった(p=0.14)。ET-1値と年齢・屈折・眼圧値・視野病期との相関を調べたところ、ET-1値はいずれ有意な相関は示さなかった(p>0.1)。ET-1値は従来の報告のように、高齢の開放隅角緑内障患者の病因とは関連あるかもしれないが、若年緑内障患者においては関連は少ないと思われた。

If the cause of cataract in normal eyes of the age group was clarified, the report included Endothelin-1(ET-1), and the increase in antibodies to Endothelin-1 was reported. The relationship between the pathogenesis of cataract in normal eyes and the mechanism of neuroimmunology was discussed. 31 cases of cataract (NTG)(mean age: 49.5 years, refractive value: -4.6 $> 3.1 diopter),18 cases of primary open corner cataract (POAG)(mean age: 44.7 years, refractive value: -8.8 $> 3.1 diopter),17 cases of normal subjects (mean age: 44.5 years). ET-1 in the upper half field was 1.2±0.5pg/ml. ET-1 in one side and lower half of visual field was 1.7±0.8pg/ml, while ET-1 in intentional difference was 1.7 ±0.8pg/ml (p=0.14). ET-1 value: year, inflection, eye pressure, visual field disease period, correlation, ET-1 value: intentional correlation (p>0.1) ET-1 is reported to be related to the etiology of cataract patients with high degrees of blindness.
