若年例の正常眼圧緑内障の原因を解明するために、これまでに報告されているEndothelin-1(ET-1),自己免疫ニューロパチーで抗体価の上昇が報告されている抗ガングリオシド抗体を同時測定し、正常眼圧緑内障の病因としての神経免疫学的機序の関与について検討した。まず、正常眼圧緑内障(NTG)31例(平均年齢49.5才、屈折値-4.6±3.1diopter)、原発開放隅角緑内障(POAG)18例(平均年齢44.7才、屈折値-8.8±3.1diopter)、正常人17例(平均年齢44.5才)に対して視力検査・視野検査・ET-1,ガングリオシド抗体の測定を行った。NTG患者、POAG患者、正常人におけるET-1値はそれぞれ、1.5±1.7pg/ml、1.6±1.8pg/ml、1.4±1.1pg/mlであった。NTG患者とPOAG患者を比較したところ、年齢・視野病期・ET-1に有意な差はなかったが、POAG患者がより近視であった(p<0.001)。抗ガングリオシド抗体は、抗GA1 IgG抗体、抗GQ1b IgG抗体、抗GM1 IgG抗体(+),抗GM2 IgG抗体(+)がそれぞれ1名ずつの異常値を認めたが、いずれも軽度陽性であり、特徴的な視神経所見もなく、特異的なものではないと考えた。全症例のうち上半視野の欠損しているものは8例、下半視野の欠損しているものが12例であった。上半視野症例でのET-1は1.2±0.5pg/mlであった。一方、下半視野症例でのET-1は1.7±0.8pg/mlであり、有意な差はなかった(p=0.14)。ET-1値と年齢・屈折・眼圧値・視野病期との相関を調べたところ、ET-1値はいずれ有意な相関は示さなかった(p>0.1)。ET-1値は従来の報告のように、高齢の開放隅角緑内障患者の病因とは関連あるかもしれないが、若年緑内障患者においては関連は少ないと思われた。

NTG patients, POAG patients, ET-1 patients in the wild disease stage, POAG patients, and POAG patients were more likely to have poor myopia than those in the wild stage. Anti-GA1 IgG antibody, anti-GQ1b IgG antibody, anti-GM1 IgG antibody (+), anti-GM2 IgG antibody (+), anti-HIV antibody (+). In all cases, there were 8 cases in the upper field and 12 cases in the lower half of the field. In the upper half of the disease, ET-1 was 1. 2 ±0.5pg/ml and the disease was severe. The patients with one side and lower hemi-vision field were diagnosed as ET-1 (1.7 ±0.8pg/ml) and intentionally (0.14). During the period of wild illness, ET-1 suffered from a variety of diseases, while those with ET-1 symptoms showed signs of illness. ET-1 reported that the cause of cataract in the corner of the hospital, the cause of cataract, the cause of cataract and the cause of cataract.
