本研究では、診療ガイドラインの機械可読形態の開発のために、病院情報システムにおける指示作成の原情報とする意図に基づく視点で診療ガイドラインの構造を分析し、機械解釈可能形態の開発を行い、この形態に基づく指示作成のための機能を暫定的に定め、病院情報システムの原型システムとの連携をおこなうことを、当該研究費申請当初の段階で計画していたが、これらの研究を実施し、機能実現が可能であることまでを示した。本研究においては、診療ガイドラインを医療機関固有の条件に合致した標準的診療として示すこと、これに基づき指示作成をおこなう方法論を実装し、検証した。この結果、診療ガイドラインを医療機関固有の条件に合致した標準的診療として示してもなお、個別の患者の診療履歴、検査結果に基づき個別化した具体的診療指針とするためには、データベースの検索にかかる応答時間と推論に要する時間が高速化される必要があると考えられ、かつ、検査結果と診療履歴の組み合わせや複数の検査結果に基づき別な指標を種々の論理により合成する必要があることが明らかとなった。今後は、標準的診療指針として一般化して記述として記される診療ガイドラインを、個々の患者の診療既往、診療計画、各種検査結果に基づいて、個々の患者に対する具体的な診療指針に自動的に変換し、かつ診療で用いられる病院情報システムの中で、実際の診療で耐える程度の応答速度を確保する技術的方法を見出し、実証することを目的として研究を推進する
