我々は進行肝細胞癌に対して少量のシスプラチンと5-FUを用いた肝動注化学療法を行い、門脈腫瘍栓合併例(Cancer 2002;95:588-595)および非合併(AJR 2003181(5):1327-34)に対して有効な治療法であることを報告してきた。しかしながら生命予後の延長が得られたのは約45%の奏功例に限られ、非奏効例の生命予後の延長は疑問であった。この臨床研究の目的は肝動注化学療法で利益の得られる患者と利益の得られない患者の識別化を治療前に行ういわゆるテーラーメイド医療の達成とした。肝動注化学療法を行った初回進行肝細胞癌患者を対象として血中及び肝細胞癌組織中の抗癌剤耐性遺伝子の発現を調べ、進行肝細胞癌に対する肝動注化学療法の良い適応症例を検討することとした。近年の乳癌や悪性リンパ腫の研究ではマイクロアレイを用いた腫瘍組織中の遺伝子発現プロファイルが患者の予後と極めて相関することが報告されている。この研究でもマイクロアレイを用いて約600個の遺伝子発現の解析を行い、血中及び肝細胞癌組織中の遺伝子発現プロファイルと臨床経過を対比する計画とした。肝臓癌モデルマウスを用いた予備実験では21G肝生検針より得られた肝臓癌組織からマイクロアレイによる遺伝子解析に十分な量のRNA量が抽出された。この研究は当院倫理委員会の承認を既に得ており現在9症例が登録され血中及び肝細胞癌組織中の遺伝子発現プロファイルを6症例について解析済みである。6例の腫瘍組織中遺伝子発現情報では正常と比べて2倍以上もしくは1/2以下の遺伝子発現を示す遺伝子が大多数を占め、進行肝細胞癌特有の遺伝子発現情報は得られそうである。臨床経過では9例中4例に奏効例が得られた。非奏効例5例中4例は既に死亡され、奏効例4例は全例生存中であり、治療効果が予後を左右する結果が得られると推測される。今後症例数の蓄積を行い遺伝子発現プロファイルと治療効果の比較検討を行い、テーラーメイド治療の達成を目標としている。
