项目摘要

N県下の歯科保健に関する地域格差や課題を明らかにするため、4つの調査と分析を行った。(1)受療調査:全歯科診療所における平成16年7月第2火曜日の受診者全数17,117人(女性57%)の年齢区分は小学生相当年齢に最初のピークがあり、55-64歳が最大であった。訪問診療は1.2%を占めたが、全受診者の25%が受診した県庁所在市19地区のうち3地区、および別の1市が全訪問診療の75%を占めており、これらの地区で訪問診療が全診療に占める割合は、11%、2.4%、1.3%、3%と、特定の地区で集中的に実施されていた。(2)歯科医師の活動調査:県歯科医師会全会員を対象とする保健活動の実情調査では、回収数25%の時点で訪問診療実施経験なしが22%、52%は訪問診療のみを実施、26%が居宅療養管理指導、摂食機能療法などを併せて実施していた。(3)幼児の生活とう蝕の関連:3歳児う蝕有病者率を市町村別にみると離島、および島原半島南部が有病率が高く、また、「近隣に歯科医院がある」、「できるだけ遅くまで甘味食を与えない」という回答者の多い地区では、う蝕有病者率が低かった。(3)離島住民の口腔保健:平成14年〜16年に離島で歯科健診を実施した1343人の約4割が未処置う蝕、15%が2本以上の未処置う蝕を有し、年齢差はなかった。65歳以上の3割以上が義歯を必要としつつ義歯を使用せず、歯が原因で不快な思いをしたことのある人の割合は歯数が少ないほど多かった。(4)児童相談所健診:N県下2箇所の児童相談所において、平成16年5月下旬以降の被虐待児を含む一次保護対象者の口腔診断査を行い、10月までの対象者70名の41%に未処置歯あり、20%に痛い歯があり、14%は「歯で困った時、我慢する」と回答した。以上の結果より、離島や歯科医療の希薄な地域、保健習慣不良、被虐待児など重点的に取り組む対象者がうかびあがり、訪問診療などの実施状況にも地域格差のあることが明らかとなった。

(3)The relationship between the life of children and the eclipse:3. The incidence rate of children's eclipse is high in cities, villages, outlying islands, and the southern part of the Shimabara Peninsula. The incidence rate is high in neighboring dental hospitals. The incidence rate is low in many areas. (3)Oral health care for residents of outlying islands: In 2014 to 2016, 1343 people received dental care in outlying islands, and about 4 percent of them had untreated oral health care. 15% of them had untreated oral health care. 65 years of age and above, the number of teeth cut is small, and the number of teeth cut is small. (4)Children's consultation centers: In the second half of May, 2016, oral examinations were conducted on 70 subjects, including those with primary child abuse. In October, 41% of the subjects did not receive treatment, 20% had pain, and 14% answered,"I am slow when I am in trouble." The above results show that there is a lack of geographical access to dental care, poor health care habits, abuse, and focus on the group of patients, and a lack of geographical access to medical treatment.
