项目摘要

平成16年度は、本学附属病院のインシデントレポートシステムを活用して蓄積されたインシデント事例集の解析を行った。そこから、明らかとなった、優先度の高い事例をもとに、教職員リスクマネージメント研修会のワークショップで利用する、架空のシナリオを作成した。シナリオをもとに、ワークショップ形式の教職員の研修活動に利用し、そこで挙げられたシナリオの問題点をフィードバックして、完成度を高めた。第二段階として、ワークショツプの成果をもとにフィードバックを受けたシナリオをもとに、ビデオ脚本を作成し、院内の教職員から出演者とスタッフを募集して、事例ビデオを作製した。そのための撮影機材と編集用機材および消耗品を、本研究費から調達した。作成した事例ビデオを、中途採用の教職員のリスクマネージメント研修に活用した。研修会後のアンケート調査では、事例ビデオについては、紙シナリオによる場合よりも評価が高く、職員研修の教材として有用性が高いと判断された。平成17年度は、平成16年度に作成した事例ビデオを、研修医の初期研修や新人看護師の研修会にも活用し、研修医の医療安全への認識度を高めることにつながったことが、研修会後のアンケート調査から明らかとなった。また、卒前医学教育への利用も試みた。臨床実習前の医学生4年生に対し、平成16年度に作成した教職員研修用ビデオ教材をもとに、少人数グループ討論によるワークショップ型の授業を実施した。授業前後の試験結果からは、学生の医療安全に対する認識が高まったことのほか、医療安全に関する具体的な知識の獲得にも有効であることが確認できた。また、平成17年度も、インシデントレポートシステムの事例をもとに、シナリオ・脚本を追加し、事例ビデオの撮影を行い、計5本の事例ビデオを蓄積した。
