项目摘要

1.対象と方法本研究の対象は、平成15年3月までに大阪府A市において要介護認定を申請し、初回判定時に要支援と認定された者のうち、期間内に2回以上申請した65歳以上の者1194名である。対象者の要介護度の変化は「要支援継続」と「要介護1」に区分し、性・年齢階級別に分析した。その内容は介護認定基本調査85項目の変化であり、2回目の申請時にこれらの項目が「要介助」と判定された者の割合についてMcNemar検定を用いて分析した。また、「要支援継続/要介護1」を目的変数とし、年齢、麻痺、医療および上記検定で全ての性・年齢階級で有意であった項目を調整し、多重ロジスティック回帰分析により基本調査と目的変数との独立した関連性を調べた。さらに、「要支援継続」「要介護1」群別の訪問介護と通所サービス利用頻度との関連性についても検討した。2.結果と考察性・年齢階級別に要介助と判定された者の割合をみると、75歳未満の男性は浴槽の出入り、洗身、居室の掃除の基本ADLに関連する項目で有意に増加していた。一方、75歳未満女性および75歳以上の者は、歩行、移乗など身体機能に直接関連のある項目で有意に増加していた。これらの項目について「維持・改善」の者に比べて「低下」した者のオッズ比をみると、浴槽の出入りが9.45、洗身が3.95、居室の掃除が5.19、歩行が5.59、移乗が9.32であった。また、要介護度の変化とサービス利用との関連性は認めなかった。これらの結果は、前期高齢者は筋力トレーニングなど身体機能に直接寄与するプログラムよりも、ADL自立に向けた福祉用具等の活用方法や生活機能に即したリハビリテーション・プログラムの提供が重要であることを示すものである。介護予防の効果的な展開のためには、介護認定調査項目など既存のデータを用いることによる対象の特性に応じた目標の設定が不可欠である。
