色素添加ゼラチンを後腹膜間隙に注入した胎児の連続切断標本を作製、実体顕微鏡下で筋膜を中心に剖出を行い、後腹膜間隙・後腹膜筋膜の広がりを検索した。本年度は12胎の検索を行った。内訳は横断標本4胎、矢状断標本4胎、冠状断4胎である。得られたデータはすべて本科学研究費で購入したデジタル撮影装置にて保管し、昨年度の標本検索と合わせ総合評価を行った。標本を検索した結果、以下の所見が得られた。・左右の後腹膜筋膜で裏打ちされた後腹膜間隙は、上腸間膜動脈周囲の神経叢の尾側で交通する。・腎臓と副腎は共に腎筋膜の前葉と後葉で覆われるが、腎臓と副腎の間にも筋膜が存在し両者を分けている。・腎門と腹大動脈および下大静脈の間には多数め隔壁様の筋膜が存在し、さらにこれらの筋膜の多くは腎臓の背側へ向かう走行を呈する。この所見は、urinoma等の疾患が腎臓の背側に好発する要因の1つと考えられる。・後腹膜間隙は横隔膜下・前〜側腹壁下・鼡径部・大腿部へ連続性に広がる。ただし、腎臓と骨盤入口部の間には水平方向に広がる脇膜が存在し、この筋膜より頭側の間隙は横隔膜下および側腹部への直接的な連続性をもたない。この筋膜より尾側の間隙が横隔膜下およぴ側腹部への直接的奪連続性をもつ。腎臓と骨盤入口部の問に今回観察された筋膜の存在は、これまでその存在が報告されたことはなく、また上記の所見の如くこの筋膜の上下で周辺後腹膜間隙への連続性が異なることから、特に後腹膜め炎症や腫瘍の進展経路・腫瘍の転移経路を考える上で注目する。今後、臨床例との整合性を検討したのち、学術誌に発表する予定である。

The kidney is divided into two parts: the anterior lobe and the posterior lobe of the renal fascia. Most of the septal fascia exists between the renal hilum, the abdominal aorta, and the inferior great vein, and most of these fascia run toward the dorsal side of the kidney. The main cause of this disease, such as urinoma, is the development of dorsal kidney disease. The posterior peritoneal space is under the diaphragm, anterior to lateral abdominal wall, radial part, and thigh joint. There is a horizontal membrane between the kidneys and the pelvic inlet, and there is a direct connection between the fascia and the diaphragm. The fascia and the caudal gap form a direct connection between the diaphragm and the lateral abdomen. The presence of the fascia in the pelvic inlet is reported, and the findings are noted, such as differences in connectivity between the upper and lower parts of the fascia and the peripheral retroperitoneal space. In the future, the integration of clinical cases and academic journals will be discussed.
