项目摘要
1)ラット大脳皮質切創損傷モデルで脳損傷部位にゼラチンを留置し、生理食塩水(対照群、n=20)、AT III 500U/kg/day(治療群、n=20)を腹腔内圧ポンプにて持続的投与した。脳損傷作成1、3、5、7日後、ゼラチンと創縁における炎症反応とダリア反応をHE染色ならびにmonocytes/macrophages (Mo/MΦ)、GFAP、vimentinによる免疫化学染色で組織学的に比較検討を行った。【結果】治療群は対照群に比較して、損傷後1、3日目における多核白血球と3、5、7日目におけるMo/MΦ陽性細胞を有意に抑制した(p<0.05)。また、GFAP陽性アストロサイトの増殖は認めなかったが、3、5、7日目におけるvimentin陽性アストロサイトの増殖を有意に抑制した(p<0.05)。以上の結果からAT IIIは、脳損傷部位近傍における炎症性細胞の浸潤やvimentin陽性アストロサイトを抑制することにより、脳損傷後の組織反応や修復過程で生じる瘢痕化を抑制する可能性が示唆された。2)脳動脈瘤破裂によるくも膜下出血患者18人ならびに未破裂脳動脈瘤患者4人において、血漿のL-セレクチン、P-セレクチン、E-セレクチン、ICAM-1、VCAM-1、MCP-1、高感度CRPをくも膜下出血発症2日(48時間)以内と7日後、未破裂脳動脈瘤患者では術前と術後7日目に測定した。対象をA群(SVS+, AVS+)4人、B群(SVS-, AVS+)3人、C群(SVS-, AVS-)11人、D群(未破裂脳動脈瘤患者)4人に分けた。SVS (symptomatic vasospasm :脳血管撃縮による脳虚血症状が出現)、AVS (angiographical vasospasm :脳血管撮影またはCTAの所見で50%以上の脳血管撃縮があり)。A群、B群、C群はD群に比べて血漿のL-セレクチン、P-セレクチン、E-セレクチン、ICAM-1、VCAM-1、MCP-1、高感度CRPが有意に高かった。また、A群はB群、C群に比べて発症2日(48時間)以内のトセレクチンと発症2日(48時間)以内と7日後のVCAM-1が有意に高かった。白血球の捕捉・遊走による循環障害、白血球の脳内浸潤による脳組織障害によって、<も膜下出血後に脳虚血症状が出現した可能性が示唆された。
