项目摘要

1.厚生労働省の、平成4〜14年度の医師歯科医師薬剤師調査の結果から、歯科医師の施設・業務の種別等について分析した。女性歯科医師の就業状況を、平成4年と14年とで比較すると、診療所や病院勤務者の割合は増加しているが、診療所の開設者は31.1%から25.6%に減少していた。一方男性歯科医師は、診療所の開設者が多く、平成4年は65.2%であったが、平成14年は70.0%に増加していた。歯科大学の所在地別にみると、平成14年の女性歯科医師数15,678人中74.3%が歯科大学所在の都道府県に集中しており、男性の69.2%を上回った。中核市・政令指定都市についてみるとこの傾向はさらに顕著で、女性歯科医師の80.7%が歯科大学のある市に集中しており、男性は74.6%であった。また、昭和50年を100として平成14年までの指数を算出すると、男性が198であるのに対し、女性は334と3倍以上に増加していた。2.50歳前後の女性歯科医師432名を対象に、就業状況や勤務状況等に関する調査を行った。有効回答数は251名(58.1%)であった。就業状況では、診療所を開業している者は59.4%であり、勤務者は23.9%、無職者は9.9%であった。結婚と出産・育児経験については、78.8%が結婚しており、77.9%の者が出産・育児経験有と回答していた。出産・育児のための休業期間については、勤務医は「1〜5年」が最も多く、38.0%で、1年以上の休業期間は8割以上であったが、開設者は「6か月未満」が43.2%と勤務者に比べて少なかった。就業状況と結婚・育児経験の有無による労働状況の違いでは、1週あたりの労働目数は結婚の有無に関係なく、開設者、勤務者ともに「5日」が最も多く、平均すると勤務者は4.5日、開設者は5.2日であった。引退予定年齢については、開設者は「61〜65歳」、勤務者は「56〜60歳」が最も多かったが、平均値に差は見られなかった。また現在無職者と離職経験者の離職の理由としては、67.4%が「子供ができたから」であり、10.9%が「健康上の問題」、2.2%が「要介護者がいるから」であった。
