本研究は、新卒看護師の臨床実践能力獲得のプロセスについて、看護実践に伴うリフレクションの実態から質的記述的に構造化し、新卒看護師教育における教育的アプローチの示唆を得ることを目的とした基礎的研究である。今年度は2003年4月に入職した新卒看護師12名に対し、半構成的面接法による調査を2003年9月から10月に実施した。調査内容は、対象者が経験した看護実践での重要な出来事や特徴的な場面について、Gibbsのリフレクティブサイクルに基づき面接調査を実施した。会話記録は対象者の承諾を得て録音し逐語記録に起こした後、質的記述的に分析した。倫理的配慮として調査対象者に「研究の全容と研究参加の自由意志の保障および対象者の匿名性の保障」を説明し、文書による研究協力の承諾を得た。分析の結果、新卒看護師のリフレクションの実態として、看護実践場面をどのようにしたら改善できるのか【結論】と、次に似たような状況で何をするのか【行動計画・指針】が詳細かつ具体的な新卒看護師と、【結論】が「(点滴の管理は)こまめに見に行く」や【行動計画・指針】が「少しでも不安があったら先輩に聞く」など、具体的な表現に至らない新卒看護師の特徴が明らかとなった。その背景には、先輩看護師や師長から促される場面の振り返りや、その看護実践に関する自己学習、自己学習できる環境などが関与していた。以上のことから、新卒看護師が看護実践において行動計画や指針を具体的に得るためには、その場面における看護の意味や重要性を実感的に理解することが重要であり、先輩看護師および師長が経験に基づく「学びを引き出す」関わりを行うことで、新卒看護師が客観的にかつ実感のある学びが得られると考える。今年度は新卒看護師の語りからリフレクションの場面における特徴を示した。今後、データを縦断的に蓄積し、新卒看護師の患者の把握、看護実践の評価や分析の特徴を明らかにし、新卒看護師のリフレクションの構造化に向けて、分析を重ねる。
