本研究の目的は,長期コホートにより,高齢者の咬合状態の悪化が運動機能(体力)低下をきたすという仮説を証明することである。新潟市在住のベースライン時(平成10年)70歳であった高齢者600名を対象として,5,6および7年後の追跡調査を行った。咬合状態の評価はEichnerの分類,体力は握力,脚伸展力,脚伸展パワー,ステッピング回数および開眼片足立ち時間について調べた。本年度の調査参加者は406名で,参加率は67.7%であった。両年度ともに調査に参加した対象者について各体力測定項目について比較した。握力は30.0±8.1kgで,ベースライン時の32.4±8.8kgと比較して有意な低下が見られた。同様に脚伸展力(58.4±20.4kg vs.52.7±16.6kg),脚伸展パワー(705.5±271.1W vs.534.2±253.1W)および開眼片足立ち時間(31.9% vs.13.3%,120秒以上のものの割合)も有意な低下が見られた。しかし,ステッピング回数は有意に増加した。つぎに,Eichnerの分類を両年度について比較したところ有意に悪化し,悪化したものは135名(32.7%)であった。そこで体力測定項目のうち有意な悪化が見られた握力,脚伸展力,脚伸展パワーおよび開眼片足立ち時間と咬合状態の悪化についての関連性を検討したが,有意な関係は認められなかった。以上より,本研究期間において,仮説を証明するには至らなかった。しかし,ステッピングを除いた体力項目は咬合状態と同様に確実に経年的悪化を来しているものの,ベースライン時に参考値として測定した当時80歳の集団と比較すれば,いずれの項目も依然悪化の程度は低い。したがって,本研究は少なくとも平成20年まで継続したいと考えている。また,ステッピング回数は経年的に明らかに増加しており,検査方法として適切であるかどうか,再検討の余地があると考えられた。

The purpose of this study is to provide long-term training for people with high levels of occlusal status and low mechanical performance (physical strength). In Xinxiang City, there were 600 people living at the age of 70 in Xinxiang (10 years in Pingcheng), and they were chasing after seven years later. Occlusal Eichner classification, physical grip strength, foot stretching strength, foot stretching strength, pedicle back count, open eye slices are sufficient to make sure that the time is sufficient.
