平成16年度における研究目的は、在宅で療養生活する高齢者、特に定期的に医療機関外来に通院している高齢者について、潜在的な脱水状況と脱水リスク要因(性、年齢、疾患、使用薬剤、健康状態、日頃の飲水状況等)ならびに脱水状態に関する自覚症状との関連性を検証することを目的とした。対象者の定期的な医療機関への通院目的は、内科的治療、脳血管障害等のリハビリテーション、骨・関節等の治療等であった。研究対象者は、研究協力の得られた都内H市のE病院の外来へ、一定の調査期間において、65歳以上の定期的な外来受診を継続している高齢者のうち研究協力の同意を得た者であった。調査項目は、血液検査(血清ナトリウム,血清カリウム、血清クロール、尿素窒素,クレアチニン、空腹時血糖、血清アルブミン、血清浸透圧)、問診内容(属性,現疾患,服薬等,飲水習慣、自覚症状)とした。対象者は159名(男性70名、女性89名)、平均年齢は74.7±6.7歳であった。血清ナトリウム値は、145mEq/lより高値の者が1名認められた。血清浸透圧値は、295mOsm/lより高値の者が31名(19.5%)認められた。300mOsm/L以上の高値の者は9名(5.7%)認められた。特に9名中、高血糖者が6名(66.7%)おり、血清ナトリウム値も140mEq/l以上者が8名(88.9%)であった。脱水状態に関連する症状・徴候では、「身体の倦怠感」「意欲ややる気の消失傾向」「口腔内の乾燥感」「のどの乾燥感」「口唇の乾燥感」の順で高頻度が認められた。飲水習慣では、毎食の食事において水分をとらない者が平均7%にみられた。食間のおいては、飲まない傾向が増加し、平均18%にみられた。脱水予防の対策として、「夏他の季節より多めに水分をとっている」と回答した者は68%にとどまり、約30%の者は夏場の意識的な水分摂取をしていない状況がみられた。
