今年度は、主に(1)未治療・医療中断者の精神障害者及び家族への援助について都内公的機関に勤務する保健師9名からインタビューした10事例に対し、保健師の判断及び援助について分析の継続、(2)(1)のデータを補足する位置付けで、研究者と保健師数名による未治療・医療中断の精神障害者及び家族に援助した4事例についての事例検討を行った。(3)さらに、かつて未治療・医療中断をしていたが現在は保健師をはじめとした精神保健福祉支援システムに安定して繋がっている3家族へのインタビューを行い、(1)(2)の保健師の行った援助から抽出された分析結果についての検証を行った。以上の3点を主に実施した。保健師からのインタビューデータ及び事例検討の内容の現時点での分析では、保健師は、未治療・医療中断者への援助について、過去の体験の受け止め、本人や家族が援助場面に登場してきたときにタイミングを逃さず関わること、関係が切れないように必ず次回に繋げる援助を提供していることなどを大切にしていることが挙げられた。過去の医療体験がその精神障害者や家族にとって外傷体験となっている場合には、上記の点について特に重点が置かれていた。また保健師がケアを提供する際、家族に代わってどこまで代行するのか、本人の気持ちと家族の気持ちが異なっている場合に保健師としてどちらの側に添った援助を展開していくのかについては、本人や家族状況をアセスメントする力、保健師自身の信条、職場のフォロー体制などの要因に影響されることが伺われた。家族へのインタビューの結果からは、家族の疾患に対する知識や医療者に対する気持ちなどが、本人の受療状況に影響しており、本人だけでなく家族に向けても、気持ちや理解について確認し、早期から教育的に関わっていく必要性があると思われた。今後も、さらに整理をしていくため、データの分析について継続していく予定である。
