本研究は継続的に心療内科で心身医学的看護を実践してきた看護師を対象に調査を行い、心療内科でのケア提供に必要な看護実践能力とその獲得プロセスを記述することを目的とする。そこで、今年度はその看護実践能力を探求する上での基礎的研究として、心療内科における看護実践の構造を明らかにする研究に取り組んだ。【目的】継続的に心療内科で心身医学的看護を実践してきた看護師(准看護師を含む)への調査を通し、心療内科における看護実践の構造について記述する。【対象】心療内科を専門とする病院に勤務する看護師もしくは准看護師16名。【方法】半構成的面接による質的記述的研究法を用いた。面接は研究者が作成したインタビューガイドをもとに半構成的にすすめ、データ分析は逐語録をもとに看護実践の内容を示す部分の比較・検討と繰り返し、カテゴリーを抽出した。【結果】心療内科における看護実践としては、<ラポールの形成をめざしたケア><自己の受け入れと適切な対処行動の獲得をめざしたケア><本来の場所へ帰ることをめざしたケア><基本的看護ケア><チームアプローチ>の5つのカテゴリーが抽出された。5つのカテゴリーのうち、<ラポールの形成をめざしたケア><自己の受け入れと適切な対処行動の獲得をめざしたケア><本来の場所へ帰ることをめざしたケア>は、心療内科における看護実践の中核を成すカテゴリーであった。また、<基本的看護ケア>は看護実践の基盤となっているカテゴリーであり、<チームアプローチ>は中核カテゴリー・基盤カテゴリーが包含される、看護実践のより効果的な展開を促進する意味合いを持つ支持的カテゴリーであった。【今後の方向性】今回の研究により明らかになった看護実践をもとに、そのケア提供に必要となる看護実践能力及びその獲得プロセスを検討していくことを今後の課題とする。
