项目摘要

平成19年度は以下の点を明らかにした。血圧日間変動および血圧短期変動の相互の関連岩手県大迫町の35歳以上の一般住民1125人(平均年齢60.7歳、女性67.1%)を対象に、約1ケ月間の家庭血圧測定、および24時間自由行動下血圧測定を施行し、血圧値の個人内標準偏差(SD)を血圧日間変動および血圧短期変動の指標として、両者の規定因子を検討した。また、心拍変動についても、心拍数の個人内標準偏差と定義した。血圧レベル、心拍レベル、心拍変動および各種危険因子(性別、年齢、肥満、喫煙、飲酒、糖尿病、高脂血症、脳心血管病既往、降圧治療有無)をモデルに投入したステップワイズ重回帰分析で比較検討した。血圧レベルは血圧日間変動および血圧短期変動における最も重要な規定因子であった。血圧変動の指標としてSDを採用した場合には、血圧レベルの補正が必須であることが改めて示された。心拍変動は血圧日間変動において正の関連があり、また主要な規定因子であった。血圧日間変動を圧受容器反射機序のみにより説明することは困難であることが示唆された。その一方、血圧短期変動では心拍変動は弱い関連にとどまった。降圧療法は血圧日間変動の弱い規定因子であったが、血圧短期変動とは殆ど関連がなかった。血圧日間変動と血圧短期変動の関連は有意であるが弱い関連であり、異なる意義を持つ血圧変動であることを明らかにした。

The following points are clear for the 19th year of Heisei. The correlation between daytime blood pressure fluctuation and short-term blood pressure fluctuation was studied for 1125 general residents aged 35 years and older (average age: 60.7 years, female: 67.1%) in Iwate Prefecture.
