本研究は乳癌に罹患者の因果関係観や心理的特性が、退院後の予後や抑うつ状態等とどのように関わるかを検討することを目的とする。岐阜市内総合病院の乳腺外科に入院中の乳癌罹患者63名を本研究の対象者とした。対象者の年齢やその他の背景情報を質問票にて取得した。がんの診断および進行度はカルテ閲覧により確認した。更に対象者に個別のインタビューを行い、個人の行動を統制する主体がどこにおかれているかという患者の信念体系をHealth Locus of Control(HLC)を用いて測定した。また個人の性格の特性として、自尊心及び楽観主義の度合いを測定した。更に、自己の疾病罹患に関する因果関係観について、19の項目別に4段階のスケールで測定した。この項目には乳癌の危険因子としてエビデンスのあるものと、危険因子以外の要因を含めた。その後乳癌摘出術を終えて退院約二ヶ月後に、心的健康度の指標となるGeneral Health Questionnaire(GHQ-28)、及び抑うつ度を測定するCenter for Epidemiological Studies Depression Scaleを用いて、疾病への心理的適応度を推定した。ベースライン時の楽観主義の度合いと、心理的適応度の各指標との間には一貫して負の関係がみられ、心理的適応度の高まりが示唆された。また自尊心の度合いと心理的適応度の指標との間にも同様に負の関係がみられた。HLCで測定された「偶然」及び「超自然」への帰属の高まりと抑うつ度との間に正の関係がみられた。19項目の因果関係観については、一定の項目において高い頻度で因果関係観が形成される傾向がみられた。またこれらの項目は、心理的適応度の高まりが示唆されるもの、低下が示唆されるもの、強い関連が見られないものに分類された。本研究は、乳癌患者を罹患時より追跡することにより、入院中に測定された個人の性格や因果関係観の形成と退院後の疾病への心理的適応度との関連を、前向きに検討した。心理的適応度の高まりが示唆された要因については、今後更なる検討を通し、医療従事者及び乳癌罹患者自身の知識として有用であることが示されよう。
