本研究の目的は、日本の医療機関における患者相談窓口を中心とした、患者とその家族の意思決定支援システムの現状を明らかにし、患者と家族にとって有用な意思決定支援システムのあり方を検討することである。本研究では、徳に、患者の診療過程にわたって接する機会の多い看護職者の役割に焦点をおく。平成17年度に文献検討を行った結果に基づき、平成18年度は訪問調査の対象医療機関を選定し、調査を実施した。医療機関は、中部地方にあるある総合病院で、対象者は、患者相談窓口を10数年に渡り担当している専任看護師である。研究者が医療機関に出向き、普段患者相談業務を行っている部屋にてインタビューを実施した。調査内容は、患者相談窓口の担当職種と構成,活動状況として窓口の営業時間と曜日,現在までに寄せられた相談内容とその件数,相談内容に患者と家族が治療や療養を意思決定することに関わるものがあるか,意思決定に関わる相談への対処方法とその成果,看護職者が果たした役割,相談への対処の中で困難を感じていることとした。インタビューは90分に渡って実施され、具体的な事例に基づいた詳細な活動に関する情報を収集することができた。この訪問調査にて得られた患者相談窓口における患者と家族の意思決定に対する看護職者の活動を基準とし、特定機能病院の患者相談窓口ではどの程度まで意思決定支援が実施されているのか、全国調査に向けデータ収集の内容および方法について検討し質問紙を作成する。
