项目摘要

平成17、18年度に群馬県倉渕村(現高崎市倉渕町)で行われた基本健康診査を受診し、本研究に同意した者(平成17年度:1412名、平成18年度:1058名)を対象に、基本受診項目に加え、スポット尿を採取し、尿中ナトリウムとカリウムおよびクレアチニンを測定し、Tanakaらの推定式(Tanaka T et al.J Hum Hyp2002)より1日あたりのナトリウム、カリウム、塩分摂取推定量を求めた。質問票により減塩行動段階を昨年と同様に5つのステージ(維持期、行動中、準備期、静観、静観以前)に分類した。解析の際は、3つのステージ(維持期+行動中、準備期、静観+静観以前)にまとめ、各年度の各ステージ別の1日あたりの尿中塩分排泄推定量を求め、時間断面的に解析を行った。さらに、平成17年度のステージが1年後の尿中塩分排泄量に反映されているかどうか調べるため、両年度ともに受診した参加者899名において、平成17年度のステージ別に平成17、18年度の尿中塩分排泄量の変化率の解析をKruskal Walllis検定により行った。各年度のステージ別尿中塩分排泄量(中央値)は、平成17年度(静観以前+静観:9.7g(男)/9.2g(女)、準備期:10.Og/9.7g、行動+維持期:9.4g/9.4g)、平成18年度(静観以前+静観:10.1g(男)/9.4g(女)、準備期:9.3g/9.5g、行動+維持期:9.7g/9.4g)間に統計学的な有意差は認められなかった。平成17年度のステージ別の尿中塩分排泄量変化率は、静観以前+静観で男女それぞれ+3.4%/-4.3%、準備期では、-4.2%/-1.3%、行動+維持期では、+2.7%/-3.5%であり(P値:男性0.086、女性0.778)統計学的に有意な差は認められなかった。行動+維持期の塩分排泄量が他のステージと比して既に低い傾向にあり、変化率にも差が無かったことより現在も減塩行動を継続していることが示唆された。また、準備期の男性の変化率はマイナスであり、減塩行動を実行していることが示唆された。しかし、いずれも統計学的に有意な差は認められず、本研究では、食行動変容段階の主観評価と客観指標の尿中塩分排泄量との一致性を明確に示すことができなかった。
