本研究は、在宅における終末期がん患者の家族の生活の質の向上を図るための看護介入方法論の開発を目的としている。平成18年度は、データ収集、分析、研究結果の発表を行った。対象者は在宅で生活する終末期がん患者の主たる介護者である家族とし、在宅ケアの安定期の家族で、インタビューが可能と判断できるがん患者の8家族であった。データ収集方法は、インタビューガイドに基づき面接調査法にて行った。面接時間は、対象者一人当たり20〜75分で、平均は40分程度であった。インタビューガイドの内容は、終末期がん患者と在宅で生活するようになってからの家族の関係性や役割の変化の有無とその内容、終末期がん患者と在宅で生活するようになってからの家族の日常生活の変化とその対応であった。データ分析は、得られたデータを逐語録にし、意味・内容が類似するものについてコード化、カテゴリー化し内容分類を行った。家族の関係性については、〔主たる介護者-患者間〕〔主たる介護者-他の家族員〕〔他の家族員-患者間〕〔家族員同士〕〔介護者-医療者間〕の5つの視点から関係性について分類した。また、《生活のあり方》《患者を中心とした変化》《介護》の3つに大別でき、《生活のあり方》では、<訪問看護者との兼ね合いの中で生活><患者の生活リズムに合わせながらの生活><自由がきかない中で生活>の3つのカテゴリー、《患者を中心とした変化》では、<患者のことが頭から離れない中での生活><その日がくるという覚悟をしながらの生活><患者の生活の尊重>3つのカテゴリー、《介護》では、<介護の方法を覚えながらの生活><家族で介護するしかない>の2つのカテゴリーが抽出された。今後は、さらに対象者を増やし普遍性の検証を行っていく。
